血清CEA、CA125、CA199及血浆M2-PK联合检测胃癌的诊断价值研究

Diagnostic value research of combined detection of serum CEA,CA125,CA199 and plasma M2-PK in gastric cancer

目的研究血清癌胚抗原(CEA)、糖链抗原125(CA125)、糖链抗原199(CA199)及血浆M2型丙酮酸激酶(M2-PK)联合检测胃癌的诊断价值。方法 39例胃癌患者作为胃癌组,同期收治的39例良性胃部疾病患者作为良性组,同期39例体检健康者作为健康组,比较三组血清CEA、CA125、CA199、M2-PK水平及其阳性情况,比较4项指标单独检测和联合检测的特异性与敏感性。结果胃癌组CA125、CEA、M2-PK、CA199水平高于良性组和健康组,差异均有统计学意义(P<0.05)。胃癌组CA125阳性率、CEA阳性率、M2-PK阳性率、CA199阳性率均高于良性组和健康组,差异均有统计学意义(P<0.05)。4项联合检测的敏感性92.31%均优于单独检测的CA125的56.41%、CEA的51.28%、M2-PK的71.79%、CA199的48.72%,差异具有统计学意义(P<0.05)。结论 CEA、CA199、CA125以及M2-PK联合检测在胃癌的诊断中敏感性较高,具有较高的诊断价值。

0bjective To research the diagnostic value of combined detection of serum carcinoembryonic antigen （CEA）, carbohydrate antigen 125 （CA125）, carbohydrate antigen 199 （CA199） and plasma M2 pyruvate kinase （M2-PK） in gastric cancer. Methods 39 cases of gastric cancer were selected as gastric cancer group, and 39 cases of benign gastric diseases admitted to the hospital in the same period were selected as benign group, 39 healthy subjects were selected as healthy group at the same time, and the levels of serum CEA, CA125, CA199, M2-PK and their pos.itive condition were compared in the three groups, the specificity and sensitivity of four indicators for separate detection and combined detection were compared. Results The levels of CA125, CEA, M2-PK and CA199 in gastric cancer group were significantly higher than those in benign group and healthy group, the differences were statistically significant （P〈0.05）. The CA125 positive rate, CEA positive rate, M2- PK positive rate and CA199 positive rate in gastric cancer group were all significantly higher than those in benign group and healthy group, the differences were statistically significant （P〈0.05）. The sensitivity of the four indicators for combined detection was 92.31%, which was significantly better than CA125 （56.41%）, CEA （51.28%）, M2-PK （71.79%）, CA199 （48.72%） for separate detection, the differences were statistically significant （P〈0.05）. Conclusion The combined detection of CEA, CA199, CA125 and M2-PK has high sensitivity in the diagnosis of gastric cancer, and has high diagnostic value.
