摘要
目的 Meta分析评价缺氧诱导因子-1α(hypoxia inducible factor-1α,HIF-1α)与中国胃癌患者的病理学特征的关联。方法检索Pubmed、中国知网(CNKI)、万方医学数据库和维普全文期刊中文数据库,获取已发表的所有关于HIF-1α蛋白的表达与中国胃癌患者病理特征的相关研究文献。应用STATA10.0SE软件,进行不同组间HIF-1α表达比值比(odd ratio,OR)、95%可信区间(95%confidence interval,CI)以及文献异质性和偏倚评估。结果按照纳入标准,本文共纳入52篇文献;胃癌组织中HIF-1α蛋白的表达与肿瘤浸润程度(OR=3.96,95%CI=3.32~4.72)、淋巴结转移(OR=3.88,95%CI=3.11~4.83)、远处转移(OR=4.05,95%CI=2.33~7.05)、肿瘤的大小(OR=1.56,95%CI=1.16~2.11,P=0.004)、肿瘤的分化程度(OR=1.54,95%CI=1.23~1.92,P=0.002)以及TNM分期(OR=3.36,95%CI=2.86~3.96)均有相关关系。结论 HIF-1α蛋白可作为对胃癌患者的早期发现、早期诊断及早期治疗有指导价值的生物学指标,也可用于预测中国胃癌患者侵袭转移和预后评估的分子标记物。
Objective This study evaluated the clinical significance of hypoxia inducible factor-1 ot (HIF-1α) expression in Chinese patients with gastric cancer. Methods To retrieve all published literatures about the expression of HIF-1 alpha and the pathological features of Chinese gastric cancer patients by retrieving Pubmed, Chinese knowledge network, Wanfang medical database and the Chinese database of VIP full text journal. Using STATA10.0SE software, the ratio of HIF-1 alpha expression ratio (odd ratio, OR), 95% confidence interval (95%confidence interval, CI) and literature heterogeneity and bias assessment were carried out among different groups. Results Totally, 52 eligible studies were included in the recta-analysis. Our results showed that the expression of HIF-1 α was associated with the invasion depth of tumor(OR=3.96; 95 % CI=3.32-4.72), lymph node metastasis ( OR=3.88; 95% CI=3.11-4.83), distant metastasis (OR=4.05 ; 95% CI=2.33-7.05 ), dedifferentiation ( OR = 1.54; 95% CI= 1.23-1.92), tumor size ( OR = 1.56 ; 95% CI= 1.16-2.11 ) and TNM staging(OR=3.36,95%Cl=2.86-3.96). Conclusion Our findings indicate that the HIF-1 α protein, as measured by immunohistochemistry ,can be used to predict the prognosis and potential for invasion and metastasis in Chinese patients with gastric cancer. HIF-1α may be an early valuablebiological marker in the early diagnosis and treatment for gastric cancer.
