肺痿冲剂方治疗特发性肺间质纤维化的临床效果及对凝血功能的影响

Clinical Effectiveness on Feiwei Medicinal Instant Granules in Treatment of Patients with Idiopathic Pulmonary Fibrosis and Its Effect on Coagulation Function

目的观察肺痿冲剂方治疗特发性肺间质纤维化(IPF)证属肺肾气虚兼血瘀患者的临床效果及对凝血功能的影响。方法选取2015年7月—2017年7月就诊的50例肺肾气虚兼血瘀型IPF患者作为研究对象。按治疗方法分为观察组和对照组,每组25例。两组均给予内科基础治疗,在此基础上观察组给予肺痿冲剂方,对照组给予安慰剂,疗程12周。观察两组治疗前后呼吸困难评分、肺功能、中医证候积分及凝血功能的变化。结果观察组的中医证候治疗有效率高于对照组(P<0.05)。治疗后,观察组喘息和乏力积分、呼吸困难评分及纤维蛋白原低于治疗前和对照组(P<0.05),一氧化碳弥散量实测值与预计值的百分比和6 min步行试验高于治疗前和对照组(P<0.05)。结论肺痿冲剂方能够有效缓解IPF肺肾气虚兼血瘀证患者呼吸困难症状、提高肺功能,从而改善患者的生活质量和凝血指标。

Objective To observe clinical effectiveness of Feiwei medicinal instant granules in treatment of idiopathic pulmonary fibrosis （IPF） patients with insufficiency of QI of the lung and kidney and blood stasis and its effect on coagulation function. Methods A total of 50 IPF patients with insufficiency of QI of the lung and kidney and blood stasis during July 2014 and July 2017 were randomly divided into observation group （ n =25） and control group （ n =25） according therapeutic methods. All patients received conventional therapy, and then observation group was treated with Feiwei medicinal instant granules orally, while control group was added with placebo orally, and the course of treatment was 12 weeks. Changes of dyspnea scores, lung function, TCM scores and coagulation function before and after treatment were observed in two groups. Results The overall response rates of TCM syndromes in observation group were significantly higher than those in control group （ P 〈0.05）. After treatment, in observation group, gasping, anergy, dyspnea scores and fibrinogen level were significantly lower （ P 〈0.05）, while values of percent of actual to predicting detection values in diffusion capacity of carbonmonoxide and six-minutes walking test were significantly higher than those before treatment and in control group （ P 〈0.05）. Conclusion Feiwei medicinal instant granules in treatment of IPF patients with insufficiency of QI of the lung and kidney and blood stasis may effectively relieve dyspnea symptom and improve lung function so as to improve quality of life and coagulation function.
