疏肝解郁胶囊联合地氯雷他定治疗慢性荨麻疹伴中度抑郁疗效观察

Efficacy Observation on Shugan Jieyu Capsule Combined with Desloratadine in Treatment of Chronic Urticaria with Depression

目的:观察疏肝解郁胶囊联合地氯雷他定治疗慢性荨麻疹伴轻中度抑郁的临床疗效。方法:使用焦虑抑郁量表(hospital anxiety and depression scale,HADS)选择慢性荨麻疹伴轻中度抑郁的患者164例,采用随机数字表法分为观察组86例和对照组78例。两组患者均每晚给予地氯雷他定片治疗,每天1次,每次5 mg;观察组在对照组治疗的基础上加用疏肝解郁胶囊治疗,每次0.72 g,每天2次。两组患者均治疗6周,于治疗6周及停药4周(总计10周)后评价临床疗效及不良反应,同时观察5-羟色胺(5-hydroxy tryptamine,5-HT)水平的变化情况。结果:治疗6周后,观察组有效率为84.88%,对照组有效率为71.79%,观察组有效率明显高于对照组,差异有统计学意义(P<0.05)。两组患者治疗后荨麻疹症状评分、焦虑抑郁量表(hospital anxiety and depression scace,HADS)评分较均治疗前降低,5-HT水平升高,差异均有统计学意义(P<0.05)。停药4周即第10周后,两组患者有效率均有下降,观察组下降较少,差异无统计学意义(P>0.05)。结论:疏肝解郁胶囊联合地氯雷他定治疗慢性荨麻疹伴轻中度抑郁疗效显著,且能降低复发率。

Objective： To observe the clinical efficacy of Shugan Jieyu Capsule combined with Desloratadine in treatment of chronic urticaria with mild to moderate depression. Methods： A total of 164 chronic urticaria patients with mild-to-moderate depression were selected by using HASDS. The patients were divided into observation group and control group by random number table. Patients in both groups were treated with desloratadine tablet once a day with 5 mg each time. The observation group was treated with Shugan Jieyu Capsule in addition to that of the control group,with 0. 72 g each time and twice a day. Both groups of patients were treated for 6 weeks,and the clinical efficacy and adverse reactions were evaluated after 6 weeks of treatment and another 4 weeks after drug withdrawal（ total 10 weeks）. Changes in the level of 5-hydroxytryptamine（ 5-HT） were also observed. Results： After 6 weeks of treatment,the effective rate in the observation group was 84. 88%,and the effective rate in the control group was 71. 79%. The effective rate in the observation group was significantly higher than that in the control group（ χ2= 4. 174,P〈0. 05）. After treatment,the urticaria symptoms score and HADS depression scores in both groups were lower than those before treatment,and the level of 5-HT was increased. The differences were statistically significant（ P〈0. 05）. After 4 weeks of drug withdrawal,that is,10 week later,the effective rate of both groups decreased,with the observation group decreased less. The difference was not statistically significant（ χ2= 3. 513,P〈0. 05）. Conclusion： Shugan Jieyu Capsule combined with desloratadine has a significant clinical effect on chronic urticaria with mild to moderate depression,which also can reduce the recurrence rate.
