活血益气通经汤治疗腰椎间盘突出症疗效观察

Huoxue Yiqi Tongjing Decoction in Treatment of Lumbar Intervertebral Disc Protrusion

目的:观察活血益气通经汤治疗腰椎间盘突出症的临床疗效。方法:将100例腰椎间盘突出症患者按随机数字表法分为观察组和对照组,每组50例,观察组患者采用活血益气通经汤治疗,黄芪30 g,党参15 g,香附15 g,茯苓15 g,杜仲12 g,当归10 g,丹参15 g,大黄10 g,川续断12 g,苍术10 g,桃仁6 g,僵蚕10 g,柴胡10 g,甘草3 g,山茱萸12 g,枸杞子12 g,每日1剂。连续服用1周,休息3 d,治疗30 d为1个疗程,治疗1个疗程。对照组患者采用常规治疗,7 d为1个疗程,共治疗4个疗程。观察两组患者的临床疗效、视觉模拟评分法(visual analogue scale,VAS)、临床症状、腰痛ODI评分标准(oswestry disability index,ODI)和并发症情况。结果:观察组患者有效率为92.00%,显著高于对照组患者的有效率76.00%,两组比较,差异有统计学意义(P<0.05)。治疗后两组患者VAS评分显著低于治疗前评分,差异有统计学意义(P<0.05)。治疗后观察组患者VAS评分为(1.23±0.57)分低于对照组(1.30±0.36)分,差异无统计学意义(P>0.05)。观察组患者腰疼评分低于对照组,下肢麻木、下肢放射痛及直腿抬高评分高于对照组,差异有统计学意义(P<0.05)。治疗后两组患者ODI评分显著低于治疗前,差异有统计学差异(P<0.05)。治疗后观察组患者ODI评分为(9.63±2.97)分,显著低于对照组患者的(12.01±3.06)分,差异有统计学意义(P<0.05)。两组患者并发症主要有顽固腰痛、运动障碍及腰椎间盘再突出,观察组患者总并发症发生率为12.00%低于对照组患者的14.00%,差异无统计学意义(P>0.05)。结论:活血益气通经汤治疗腰椎间盘突出症疗效显著,且能缓解腰疼症状,减轻患者病痛。

Objective： To observe the clinical efficacy of Huoxue Yiqi Tongjing Decoction in treatment of lumbar disc herniation.Methods： 100 patients with lumbar disc herniation were randomly divided into observation group and control group,with 50 cases in each group. Patients in the observation group were treated with Huoxue Yiqi Tongjing Decoction： Astragalus 30 g,Codonopsis 15 g,Cyperus rotundus 15 g,Eucommia 12 g,Angelica 10 g,Salvia miltiorrhiza 15 g,Rhubarb 10 g,Rhizoma Dioscoreae 12 g,Atractylodes 10 g,Peach kernel 6 g,Bombyx mori 10 g,Radix Bupleuri 10 g,Licorice 3 g,Hawthorn 12 g,Gardenia 12 g,1 day. Take 1 week continuously,and then rest for 3 days. 30 days is one course of treatment. The patients in the control group were given routine treatment,with 7 days for a course of treatment. There was a total of 4 courses of treatment. The clinical efficacy,visual analogue scale（ VAS）,clinical symptoms,oswestry disability index（ ODI） and complications of the two groups were observed. Results： The effective rate was 92. 00% in the observation group,which was significantly higher than that（ 76. 00%） in the control group.There was a significant difference between the two groups（ P〈0. 05）. After treatment,the VAS scores of the two groups were significantly lower than before treatment,and the difference was statistically significant（ P〈0. 05）. After treatment,the VAS score of the observation group was（ 1. 23 ± 0. 57） points was lower than that of the control group（ 1. 30 ± 0. 36） points,yet the difference was not statistically significant（ P〈0. 05）. The scores of patients with low back pain in the observation group were lower than those in the control group. The scores of numbness in the lower extremities,radiating pain in the lower extremities,and straight leg raising scores in the observation group were significantly higher than that in the control group（ P〈0. 05）. After treatment,the ODI scores of the two groups were significantly lower than before treatment,and the differences were statistically significant（ P〈0. 05）. After treatment,the ODI score of the observation group was（ 9. 63 ± 2. 97） points,which was significantly lower than that of the control group（ 12. 01 ± 3. 06） points. The difference was statistically significant（ P〈0. 05）. The complications of the two groups mainly consisted of persistent low back pain,dyskinesia,and lumbar disc herniation. The total complication rate in the observation group was 12. 00% lower than that in the control group（ 14. 00%）,and the difference was not statistically significant（ P〈0. 05）. Conclusion： Huoxue Yiqi Tongjing Decoction is effective in treating for lumbar intervertebral disc herniation. It can relieve the symptoms of back pain and relieve patients＇ related pain.
