以肠套叠为首发症状的肠脂肪瘤2例并文献复习

Two cases of intestinal lipoma with intussusception as initial symptom and literature review

目的探讨肠脂肪瘤的临床表现、超声、影像学、内镜检查及病理形态学特点,以期为临床诊治提供参考。方法对1例十二指肠多发性脂肪瘤及1例结肠脂肪瘤病例进行病理学检查及文献复习。结果十二指肠多发性脂肪瘤及结肠脂肪瘤大体均表现为粘膜下肿块,病理诊断为肠黏膜下脂肪瘤,临床均表现为肠套叠。结论肠脂肪瘤临床症状无特异性,首发症状为肠套叠时易发生误诊,消化道气钡双重造影、超声内镜、B超、CT、磁共振检查有助于明确病变部位、大小、与周围器官的关系。

Objective To understand the clinical characteristics of colitis adipose tissue as manifested in ultrasonography,imaging,endoscopy and pathomorphology so as to obtain scientific knowledge for clinical diagnosis and treatment of the disease.Methods One case of duodenal multiple lipoma and one case of colonic lipoma were subjected to pathological examination and literature review.Results The duodenal multiple lipoma and colitis lipoma were mainly submucosal masses.The pathological diagnosis was intestinal submucous lipoma and the clinical manifestations were intussusception.Conclusions Intestinal lipomas bear no specificity in clinical symptom.They are prone to misdiagnoses in cases of intussusception being the initial symptom.The practices of digestive tract double-contrast radiography,EUS,B-ultrasound,CT and MR are proved propitious in determining the location and the size of pathologies and the relatedness with neighboring organs.
