[牙合]龈距离不足的磨牙应用两种材质嵌体冠修复的效果分析

The comparative analysis of two kinds of material endocrown restoration on molars with insufficient occlusal distance

目的:比较两种材质的嵌体冠修复后牙(牙合)龈距离不足的临床疗效。方法:选取磨牙缺损伴临床高度不足患者56例,共计56颗患牙;磨牙缺损分别采用CAD/CAM氧化锆全瓷嵌体冠(A组)和贵金属嵌体冠(B组)修复。在修复后2周,12个月和24个月分别进行随访观察,比较其修复效果。结果:随访观察显示,A组有2例边缘检查有缝隙,4颗邻接关系在修复后12个月和24个月检查时略松,2例边缘继发龋,4例有牙龈和牙周炎症反应,3例解剖形态稍不理想,修复体未见损坏、松动和缺失。B组3例在24个月回访时发现边缘有继发龋,2例邻接关系略松,1例解剖形态受影响,5例有牙龈和牙周炎症反应,修复体未见损坏、松动和缺失。两组嵌体冠修复效果比较,差异无统计学意义。结论:根据患牙条件综合考虑选择材质,氧化锆全瓷和金合金贵金属嵌体冠两种修复材料均可作为患者修复治疗的一种选择方案。

Objective： To compare the clinical efficacy of two materials endocrown with insufficient occlusal distance. Methods： Fifty-six patients with molar defect and clinical height insufficient that a total of 56 teeth. The molar defects were treated with CAD/CAM zirconia endocrowns（ group A） and Precious Metal endocrowns（ group B）. Follow-up observation was performed at 2 weeks,12 months and 24 months after repair,and the repair effect was compared. Results： In group A,there were 2 cases in the marginal examination,and the 4 adjacent relationships were slightly loose in the 12 th and 24 th months after the repair,2 cases were secondary to paralysis on the margin,4 cases had periodontal reaction,and 3 cases were slightly unsatisfactory in anatomy. No damage,looseness,and absence were seen in the body. Three patients in group B had secondary palpebral margins at the 24-month follow-up,2 cases had loose adjoining relation,1 case of anatomy was affected,5 case of gingival and periodontal inflammatory reaction,and no damage,looseness,and absence were seen in the prosthesis.There was no significantly difference in the effect of two materials of inlay crowns on the restoration of molar defects. Conclusion： According to the comprehensive consideration of dental conditions,the application of inlay crown simplifies the clinical operation and has a good repair effect. However,due to the limitations of the evaluation methods in this study,the long-term effects and functional status of the two materials need to be evaluated in various aspects.
