口腔门诊成人患者牙科焦虑临床相关因素的分析

Analysis of clinical related factors on dental anxiety in adult dental outpatients

目的探讨口腔门诊成人患者牙科焦虑的临床相关因素。方法选择2017年6至10月,上海市徐汇区牙病防治所和复旦大学附属中山医院口腔门诊就诊的376例成人患者,采用牙科焦虑量表(DAS)评估牙科焦虑的发生情况,运用SPSS 18.0软件进行统计学分析。结果焦虑和高度焦虑患者共计50例,非焦虑患者326例,两组间文化程度(4/26,6/27,10/74,30/199)的分布差异无统计学意义(χ~2=0.83,P>0.05),年龄(27/98,12/184,11/44)、性别(42/181,8/145)、牙科治疗史(7/11,43/315)与童年牙科心理恐惧(12/128,38/198)等变量的分布差异有统计学意义(χ~2=18.37,14.57,10.74,4.32;均P<0.05)。进一步采用多因素Logistic回归,对牙科焦虑的影响因素进行关联分析,结果显示,牙科焦虑与年龄(OR=50.66,3.37)、性别(OR=65.25)、有无牙科治疗史(OR=14.22)、有无童年牙科心理恐惧(OR=3.64)相关(均P<0.05)。结论成人牙科焦虑临床常见,患者的性别、年龄、有无牙科治疗史、童年牙科心理恐惧和牙科焦虑的发生有密切关系。

Objective To explore the clinical related factors of dental anxiety in adult outpatients.Methods A total of 376 adult patients who were treated in Odontopathy Prevention Institute and the dental clinic of Zhongshan Hospital Affiliated to Fudan University from June to October 2017 were enrolled. The dental anxiety scale（ DAS） was used to assess the occurrence of dental anxiety. SPSS 18.0 software was used for statistical analysis.Results There were 50 adult outpatients with anxiety or severe anxiety and 326 cases without anxiety. There was no statistically significant difference in degree of education between the two groups（ 4/26,6//27,10/74,30/199,χ2= 0. 83,P 〉0. 05）. However,significant differences were found in the age（ 27/98,12/96,11/44）,gender（ 42/181,8/145）,dental treatment history（ 7/11,43/315） and childhood dental psychological fear（ 12/128,38/198） between two groups（ χ2= 18.37,14.57,10.74,4.32,all P〈 0.05）. Multivariate Logistic regression was used to analyze the association of factors affecting dental anxiety. The results showed that it was significantly correlated with age（ OR = 50.66,3.37）,gender（ OR =65.25）,history of dental treatment（ OR = 14.22） and childhood dental psychological fear（ OR = 3.64）（ all P 〈 0.05）. Conclusions Adult dental anxiety is common in clinical practices. There is a close relationship between the occurrence of anxiety and the patient＇ s gender,age,history of dental treatment and childhood dental psychological fear.
