双镜联合与开腹手术治疗肝外胆管结石疗效对比

Comparison of curative effect of extrahepatic bile duct stones treated by double lens combined with laparotomy

目的比较腹腔镜联合胆道镜(双镜)与传统开腹手术治疗肝外胆管结石的临床效果。方法随机将2012-01—2016-03间64例肝外胆管结石患者分为2组,各32例。观察组接受双镜手术,对照组实施开腹手术。比较2组手术时间、术中出血量、术后胃肠功能恢复时间、切口并发症及住院时间等指标。结果 2组手术均获成功。观察组手术时间、术中出血量、术后胃肠功能恢复时间、并发症、住院时间均优于对照组,差异有统计学意义(P <0. 05)。结论双镜手术治疗肝外胆管结石具有手术时间短、术中出血量少、术后胃肠功能恢复时间快、切口并发症少、住院时间短等优点,值得临床应用。

Objective To compare the clinical effect of laparoscopic combined choledochoscopy with traditional laparotomy in the treatment of extrahepatic cholelithiasis.Methods A retrospective study was performed to analyze the medical history of the Corps hospital from January 2012 to March 2016, during which 32 cases of extrahepatic bile duct calculi were treated by double mirror （laparoscopy combined with biliary endoscope）, and 32 cases of extrahepatic bile duct calculi were treated with laparotomy. Operation time, intraoperative hemorrhage, postoperative gastrointestinal function recovery time, incision complication and hospitalization time were compared between the two groups.Results The operation was successful in both groups, and there was no conversion to open operation in the observation group. The time of operation, the amount of blood lost during operation, the time of recovery of gastrointestinal function, the complications of incision and the time of hospitalization in the observation group were significantly better than those in the control group （ P 〈0.05）.Conclusion Double-mirror minimally invasive surgery for extrahepatic bile duct calculi has the advantages of short operation time, less intraoperative bleeding, quicker postoperative gastrointestinal function recovery time, less complication of incision and short hospitalization time, and is worth publicizing by medical staff .
