关爱随访对脑卒中后抑郁患者抑郁情绪和日常生活能力的影响

Effect of caring follow-up on depression emotion and daily viability in poststroke depression patients

目的探讨关爱随访对脑卒中后抑郁(PSD)患者抑郁情绪和日常生活活动能力的影响。方法选择2016年6月~2017年9月在浙江省嘉兴学院附属第二医院神经内科住院的PSD患者79例,按照随机数字表法分为干预组39例,对照组40例。对照组采用常规门诊随访,干预组在此基础上实施为期6个月的关爱随访。2组于干预前及干预6个月采用汉密尔顿抑郁量表(Hamilton's depression scale,HAMD)及Barthel指数量表(Barthel index,BI)进行抑郁和日常生活活动能力的评定。结果 2组干预前HAMD和BI评分比较,差异无统计学意义(P>0.05)。干预组干预后HAMD评分明显低于干预前[(13.24±4.05)分vs(18.27±4.96)分,P<0.01],且HAMD评分明显低于对照组[(13.24±4.05)分vs(16.93±4.83)分,P<0.01]。2组干预后BI评分明显高于干预前,差异有统计学意义(P<0.01)。干预组干预后BI评分明显高于对照组[(80.16±9.82)分vs(74.58±8.23)分,P<0.01]。结论关爱随访不仅可有效缓解PSD患者的抑郁情绪,还能明显提高PSD患者日常生活能力,值得推广应用。

Objective To study the effect of caring follow-up on depression emotion and daily viability in poststroke depression（PSD）patients.Methods Seventy-nine PSD patients admitted to our hospital were divided into intervention group（n=39）and control group（n=40）.The patients in control group were followed up at outpatient department while those in intervention group received caring follow-up for 6 months.Their depression emotion and daily viability were scored according to the Hamilton＇s depression scale（HAMD）and Barthel index（BI）before intervention and after 6 months of intervention.Results No significant difference was found in HAMD score and BI between the two groups before intervention（P〉0.05）.The HAMD score was significantly lower in intervention group after intervention than before intervention（13.24±4.05 vs 18.27±4.96,P〈0.01）and in intervention group than in control group after intervention（13.24±4.05 vs16.93±4.83,P〈0.01）.The BI was significantly higher in two groups after intervention than before intervention（P〈0.01）and in intervention group than in control group after intervention（80.16±9.82 vs 74.58±8.23,P〈0.01）.Conclusion Caring follow-up can not only alleviate the depression emothion of PSD patients,but also improve their daily viability,and is thus worthy of popularization in clinical practice.
