乳腺导管原位癌与浸润性癌生物学状态的比较

COMPARISON OF BIOLOGICAL STATUS BETWEEN DUCTAL CARCINOMA IN SITU AND INVASIVE CARCINOMA OF THE BREAST

摘要 目的探讨并比较乳腺导管原位癌与浸润性癌的生物学状态特点。方法回顾性分析2011年1月—2016年1月就诊于我院的乳癌病人的临床病理资料,包括乳腺单纯导管原位癌300例,乳腺导管原位癌伴浸润性癌300例。分别对乳腺导管原位癌伴浸润性癌中的两部分(分别简称为导管原位癌部分和浸润性癌部分),乳腺单纯导管原位癌与浸润性癌部分的雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(Her-2)、细胞增殖指数(Ki-67)进行比较。结果导管原位癌部分与浸润性癌部分的ER、PR阳性表达差异无显著性(P>0.05);Her-2、Ki-67阳性表达差异有显著性(P<0.05)。乳腺单纯导管原位癌与浸润性癌部分的ER、PR、Her-2阳性表达差异无显著性(P>0.05);Ki-67阳性表达差异有显著性(P<0.05)。结论乳腺导管原位癌在进展为浸润性癌的过程中生物学状态指标有不同的变化,ER、PR表达差异无显著性,Her-2、Ki-67表达有显著变化,可用以指导临床对病人进行预后评估及制定个体化治疗方案,并为乳腺导管原位癌潜在的新的治疗方法提供靶点。 Objective To investigate and compare the biological characteristics of ductal carcinoma in situ (DCIS) and invasive carcinoma of the breast. Methods A retrospective analysis was performed on the clinicopathological data of the patients with breast cancer who visited the Affiliated Hospital of Qingdao University from January 2011 to January 2016, including 300 cases of breast DCIS and 300 cases of DCIS and invasive carcinoma of the breast. The expression of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (Her-2), and cell proliferation index (Ki-67) were compared between the part of DCIS and the part of invasive carcinoma in the patients with DCIS and invasive carcinoma of the breast. Results There were no significant differences in the expression of ER and PR between the part of DCIS and the part of invasive carcinoma ( P 〉0.05); however, there were significant differences in the expression of Her-2 and Ki-67 between them ( P 〈0.05). There were no significant differences in the expression of ER, PR, and Her-2 between the breast DCIS and the part of invasive carcinoma ( P 〉0.05); however, there was a significant difference in Ki-67 between them ( P 〈0.05). Conclusion In the process of breast DCIS progressing to invasive carcinoma, there are different changes in the indicators of biological status: no significant changes in the expression of ER and PR and significant changes in the expression of Her-2 and Ki-67, which can be used to guide the prognostic evaluation and the individualized treatment and provide targets for a new potential treatment for breast DCIS.
