1565nm非剥脱点阵激光联合糖皮质激素治疗增生性瘢痕疗效观察

Efficacy of non-ablative 1565-nm fractional laser in the treatment of hypertrophic scar combined with glucocorticoid

目的:探讨分析1 565nm非剥脱点阵激光联合糖皮质激素治疗增生性瘢痕患者的疗效。方法:选取60名增生性瘢痕患者,将其随机分成对照组和观察组各30例,对照组将醋酸曲安奈德注射在瘢痕增生处,观察组则在对照组相同治疗的基础上即刻给予1 565nm非剥脱点阵激光治疗;随后观察3个月并记录患者的瘢痕变化情况,并依据温哥华瘢痕标度法(Vancouwer scar scale,VSS)进行评分,并统计、对比治疗有效率以及相关并发症等数据。结果:观察组患者的总体治疗有效率为93.4%,而对照组患者的总体有效率仅为70%,差异有统计学意义(P<0.05);经过系统治疗后,两组患者的瘢痕增生均有明显好转,VSS评分均优于治疗前,其中观察组的治疗效果更佳,两组患者治疗后的差异有统计学意义(P<0.05);研究数据显示患者在治疗期间的不良反应发生率差别不大,差异无统计学意义(P>0.05)。结论:在治疗增生性瘢痕方面应用1 565nm非剥脱点阵激光联合糖皮质激素注射治疗能够明显改善VSS评分,提高治疗有效率。

Objective： To understand 1 565nm non ablative fractional laser combined with glucocorticoid in treating hypertrophic scar.Methods： 60 patients with hypertrophic scar were randomly divided into a control group（ n = 30） and an experimental group （ n = 30）.The control group were treated with glucocorticoid injection.The experimental group were treated with 1 565nm non ablative fractional laser combined with glucocorticoid injection.The Vancouver Scar Scale （Vancouwer scar scale VSS） scores of the two groups as acquired before and after treat-ment, the rates of effective treatment and the incidences of resultant complications were analyzed.Results： Findings The total effective rate in the experimental group was（93.4%）, higher than that in the control group （70%）.The differ-ence was statistically significant（P〈0.05）.After treatment, the VSS score of the two groups was improved compared with that before treatment, with that of the experimental group being more significant than that of the control group.The difference was statistically significant（P〈0.05）.No significant difference was found between the two groups in the rate of side effects（P〉0.05）.Conclusion： 1 565nm non ablative fractional laser combined with gluco-corticoid injection in the treatment of hypertrophic scar can improve the VSS score and the treatment efficiency and thus canserve as a safe and reliable method for treating hypertrophic scar.
