电针疗法联合康复治疗对急性缺血性脑卒中患者临床疗效及神经功能的影响

Clinical Effect of Electroacupuncture Combined with Rehabilitation Therapy on Patients with Acute Ischemic Stroke and its Influence on Neurological Function

目的观察电针疗法联合康复治疗对急性缺血性脑卒中(CIS)患者临床疗效及神经功能的影响,并探讨其作用机制。方法将166例患者随机分为观察组和对照组,对照组进行常规治疗+康复治疗,观察组在对照组基础上行电针治疗;观察患者日常生活、神经和运动能力改善情况,ELISA法检测血清C反应蛋白(CRP)、白介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)含量、纤维蛋白酶原(FIB)、凝血酶原时间(PT)、凝血酶时间(TT)及活化部分凝血活酶时间(APTT)改变状况。结果治疗1、2、4周后患者m BI评分、Brunnstrom评分均高于治疗前(P<0.05),且观察组高于对照组(P<0.05);治疗1、2、4周后患者NHISS评分低于治疗前(P<0.05),且观察组低于对照组(P<0.05)。治疗1、2、4周后患者TNF-α、hs-CRP、IL-6均低于治疗前(P<0.05),且观察组低于对照组(P<0.05)。治疗1、2、4周后患者FIB低于治疗前(P<0.05),且观察组低于对照组(P<0.05);治疗1、2、4周后患者PT、TT及APTT均高于治疗前(P<0.05),且试验组高于对照组(P<0.05)。结论电针疗法结合康复治疗可明显改善CIS患者运动和神经能力,降低血清炎症因子,减少产生新血栓产生的风险。

Objective： To analyze the effect of electroacupuncture combined with rehabilitation therapy on the clinical research and neurological function in patients with acute ischemic stroke（CIS）,and to discuss the mechanism of its action. Methods： 166 cases of CIS patients were randomly divided into two groups. The control group received rehabilitation treatment,while the test group received electroacupuncture treatment on the basis of the control group. The improvement of daily life,neurological and motor ability of patients was observed. The contents of hs-CRP,IL-6 and TNF-α and the change of FIB,PT,TT and APTT were detect by ELISA. Results： After treatment for 1 week,2 weeks and 4 weeks,m BI scores and Brunnstrom scores was significantly higher than that before treatment（P〈0.05）,and the test group was higher than the control group（P〈0.05）;NHISS scores decreased significantly（P〈0.05）,and the test group was lower than the control group（P〈0.05）;TNF-α,hs-CRP and IL-6 were significantly lower than before treatment（P〈0.05）,and the test group was lower than the control group（P〈0.05）;FIB decreased compared with before treatment（P〈0.05）,and the test group was lower than the control group（P〈0.05）;PT,TT and APTT were increased compared with before treatment（P〈0.05）,and the test group was higher than the control group（P〈0.05）. Conclusion： Electroacupuncture combined with rehabilitation therapy can significantly improve the motor and neurological abilities of patients with CIS,reduce serum inflammatory factors,and reduce the risk of new thrombosis.
