摘要
目的探讨热性惊厥(febrile convulsion,FC)患儿血电解质血糖的临床监测意义。方法选取2016年6月至2017年6月在我院急诊及儿科住院部的FC患儿30例作为惊厥组,另外选取高热无惊厥的同龄患儿30例作为高热组,选取门诊体检健康的同龄儿童30例作为对照组。分别于惊厥期、惊厥缓解期、发热消退稳定期采集惊厥组血液生化样本;分别于高热期、高热缓解期、发热消退期采集高热组的血液生化样本,连续3天空腹采集对照组的血液生化样本。分别检测钾钠氯钙含量及血糖水平。结果惊厥组惊厥期的血清钾、钙浓度与高热组高热期比较无明显差异(P>0.05),但均显著低于对照组,差异有统计学意义(P<0.05);惊厥组惊厥期的血清钠浓度明显低于高热组高热期、对照组,血糖明显高于两组,差异有统计学意义(P<0.05);高热组高热期与对照组的血清钠、血糖比较无明显差异(P>0.05);三组血清氯比较无明显差异(P>0.05)。随着症状缓解至稳定期,惊厥组与高热组血清钾、钙浓度逐步升高,惊厥组的血糖降低、血清钠升高。结论在治疗热性惊厥患儿时监测其电解质及血糖水平的变化具有重要临床价值。
Objective To investigate the clinical monitoring significance of blood electrolyte glucose in children with febrile convulsion.Methods Selection in June 2016 to June 2017 in our hospital this pediatric inpatient and 30 cases of children with FC as eclampsia group,in addition to choose 30 cases of children with high fever without convulsions of the same age as high fever group,from outpatient medical health children of her own age 30 cases as control group.The blood biochemical samples of convulsion group were collected in convulsion stage,convulsion relief period and fever resolution.Blood biochemical samples were collected from high-thermal,high-heat remission period and fever recuperation period,and the blood biochemical samples were collected from the control group for 3 days.The content of potassium sodium chloride and blood glucose level were respectively detected.Results There was no significant difference between serum potassium and calcium concentration in the convulsion group(P〈0.05),and the difference was statistically significant(P〈0.05).The serum sodium concentration in the convulsion group was significantly lower than that in the high heat group and the control group,and the blood glucose was significantly higher than that in the two groups,and the difference was statistically significant(P〈0.05).There was no significant difference in serum sodium and blood glucose between the hyperthermal group and the control group(P〈0.05).There was no significant difference between the three groups(P〈0.05).As the symptoms were reduced to stable,the serum potassium and calcium levels in the convulsion group and the high fever group gradually increased,and the blood glucose in the convulsion group decreased and the serum sodium increased.Conclusion It is of important clinical value to monitor the changes of electrolyte and blood glucose level in children with febrile convulsion.
