优化气道管理预防机械通气患者呼吸机相关性肺炎的效果观察 被引量:17

摘要 目的探讨优化气道管理预防机械通气患者呼吸机相关性肺炎的临床效果。方法选择2016年1月至2017年1月在我科接受机械通气治疗的患者80例,随机分为对照组39例与研究组41例,对照组患者接受常规气道管理,研究组患者接受优化气道管理,比较两组患者呼吸机相关性肺炎发生率、口腔清洁度、护理满意度、临床指标。结果研究组患者呼吸机相关性肺炎发生率低于对照组(P<0.05),口腔清洁度高于对照组(P<0.05),患者抗生素滴注时间、感染控制时间、肺部啰音消失时间短于对照组(P<0.05)。结论优化气道管理能有效降低呼吸机相关性肺炎发生率,保持口腔清洁,缩短抗生素使用时间、肺部啰音消失时间、感染控制时间,提高护理满意度,值得临床推广使用。 Objective To investigate the clinical effect of airway management optimization to prevent ventilator-associated pneumonia in patients undergoing mechanical ventilation. Methods To select 80 cases of patients undergoing mechanical ventilation in our department from January 2016 to January2017 as study subjects,and they were divided into the control group of 39 patients and the study group of 41 patients randomly. Patients in the control group received conventional airway management,while patients in the study group received optimized airway management. The incidence of ventilator-associated pneumonia,oral cleanliness,nursing satisfaction,and clinical indicators between the two groups were compared. Results The incidence of ventilator-associated pneumonia in the study group was lower than that in the control group( P〈0. 05). The degree of oral cleanliness in the study group was higher than that in the control group( P〈0. 05). The antibiotic instillation time,infection control time,and time for the disappearance of lung rale were shorter in the study group than in the control group( P〈0. 05). Conclusion Optimizing airway management can effectively reduce the incidence of ventilator-associated pneumonia and maintain oral hygiene. Meanwhile,it can shorten antibiotic use time,lung rale disappear time,and infection control time. As a result,it improves nursing satisfaction,and is worthy of clinical promotion.
