住院老年高血压患者心理状态与治疗依从性的相关性研究

Correlation between psychological state and therapeutic compliance in hospitalized elderly hypertensive patients

目的探讨老年高血压住院患者的心理健康状况和治疗依从性。方法将2012年5月至2014年5月我院收治的200例老年高血压住院患者分为空巢老人组、丧偶老人组、五保户老人组及常规老人组,使用症状自评量表(SCL-90),纽芬兰纪念大学幸福度量表(MUNSH),社会支持评定量表(SSRS),治疗依从性问卷对所有老年患者进行调查并分析。结果老人偏执、焦虑、抑郁、人际关系敏感及躯体化症状五因子差异有统计学意义(P<0.05);不同分组患者MUNSH量表中总分、NE、PE、NA及PA评分差异有统计学意义(P<0.05);不同分组患者其客观支持及主观支持间差异有统计学意义(P<0.05)。Logistic分析结果显示,年龄及患者主动性是患者治疗依从性的影响因素(P<0.05)。结论通过研究不同人群的住院老年高血压患者的心理状况,及时给予干预和影响,提高治疗依从性,为临床医护用药和治疗提供科学合理的依据。

Objective To investigate the mental health status and treatment compliance in hospitalized senile patients with hy-pertension.Methods Two hundreds elderly patients with hypertension hospitalized in our hospital between May 2012 and May 2014 were divided into empty nester group,widowed old mfin group,infirm old infill group and regulfir old man group.The symptom self-as-sessment scale （SCL-90） , memorial university of Newfoundland scale of happiness （MUNSH） , social support rating scale （SSRS） and treatment compliance questionnaire were used.Results There were statistically significances in five factors including elderly paranoia, anxiety, depression, interpersonal sensitivity and somatization symptoms （P 〈 0.05 ） . There were also significant differences in total scores of MUNSH scale, NE, PE and PA among the groups as well as in objective support and subjective support among the groups （ P〈 0.05）.Logistic analysis results showed that age and patients’ initiative were influence factors of treatment compliance （ P〈0.05 ） . Conclusion The study on psychology of various populations of hospitalization elderly patients with hypertension, timely intervention and beneficial effect would improve the treatment compliance.It provides the scientific and reasonable basis for clinical care and drug treatment.
