PDCA循环式护理干预在过敏性紫癜患儿中的应用

Effect of PDCA Circulatory Nursing Intervention on Children with Henoch-Schonlein Purpura

目的探讨PDCA循环式护理干预在过敏性紫癜(Henoch-Schonlein purpura,HSP)患儿的应用效果。方法选取2015年3月至2017年2月于本院收治的87例HSP患儿作为研究对象。将45例实施PDCA循环式护理干预的HSP患儿设为研究组,42例实施常规护理的HSP患儿设为对照组。比较两组患儿的血清指标、HSP症状消失时间、随访一年的复发情况。结果两组护理前后比较,研究组血清免疫球蛋白(Ig A,Ig G,Ig M)、高敏C反应蛋白(hs-CRP)显著低于对照组(P <0. 05)。两组关节肿痛、尿蛋白消失时间比较,差异均无统计学意义(P> 0. 05);研究组皮肤紫癜、腹痛、消化道黏膜出血消失时间明显短于对照组(P <0. 05),研究组复发率显著低于对照组(P <0. 05)。结论 PDCA循环式护理干预能有效缩短HSP症状消退时间,改善患儿免疫功能,辅助临床疗效,降低复发率,值得临床推广应用。

Objective To investigate the effect of PDCA circulatory nursing intervention on children with Henoch-Schonlein purpura （HSP）.Methods The 45 HSP children given PDCA circulatory nursing intervention were assigned to the study group. 42 HSP children given routine nursing were set as control group. The serum markers, the HSP symptoms disappear time, and the recurrence rate during one year follow-up were compared between the two groups.Results The C3 and C4 levels between the two groups before and after nursing showed no significant difference （ P 〉0.05）; The levels of the serum immunoglobulin （IgA, IgG, IgM）,hypersensitive C-reactive protein （hs-CRP） were lower in the study group than those in the control group （ P 〈0.01）; The disappear time of symptoms in two groups, including joint swelling, pain, urinary protein, showed no significant difference （ P 〉0.05）; The incidence of the skin purpura, abdominal pain, and gastrointestinal mucosa bleeding in the study group was less than that in the control group （ P 〈 0.01）; The recurrence rate of the study group was less than that in the control group （12.20% vs 25.00%, P 〈0.05）.Conclusion PDCA circulatory nursing intervention can effectively shorten the HSP symptoms disappear time, improve the immune function, reduce the recurrence rate, which is worthy of in-depth study.
