3D腹腔镜手术与开腹手术切除胃部胃肠间质瘤的疗效观察

The effects of 3D laparoscopic surgery and open surgery for resection of gastrointestinal stromal tumors of the stomach

目的探讨3D腹腔镜手术与开腹手术切除治疗胃部胃肠间质瘤的疗效。方法收集2014年3月~2017年3月来院就诊的胃肠道间质瘤患者100例,随机分为研究组与对照组,每组各50例。研究组采取3D腹腔镜手术治疗,对照组采取开腹手术治疗,观察对比两种手术方案的手术情况、术后恢复情况、血清IL-6与CRP水平以及疼痛评分。结果两组患者手术时间比较,差异无统计学意义(P>0.05);研究组患者排气时间、术中出血量、住院时间等与对照组比较均有明显减少(P<0.05);研究组患者恢复进食时间、恢复活动时间、胃管放置时间、引流管放置时间等与对照组比较均有显著减少(P<0.05);研究组并发症发生率均低于对照组(P<0.05);研究组血清IL-6与CRP水平以及疼痛评分均显著低于对照组(P<0.05)。结论 3D腹腔镜手术治疗胃部胃肠间质瘤相对于传统的开腹手术疗效更显著,手术过程中出血量少,疼痛轻,且患者术后恢复快,预后情况更佳,可在临床推广应用。

Objective To compare the efficacy of contrast-enhanced laparoscopic surgery and open surgery in the treatment of gastric gastrointestinal stromal tumors.Methods 100 patients with gastrointestinal stromal tumors were collected from March 2014 to March 2017,and were randomly divided into study group and control group.The patients in the study group underwent 3 Dlaparoscopic surgery.The control group was treated with open surgery.The operation of the two surgical regimens and the postoperative recovery were observed and compared.Results There were no significant differences of operation time between the two groups（P〉0.05）.Compared with the control group,the intraoperative blood loss,exhaust time and hospitalization time of the study group were significantly decreased（P〈0.05）.In the study group,the time of recovery,the time of gastric tube placement and the time of drainage tube were significantly lower than that in the control group（P〈0.05）.The incidence of complications in the study group was lower than that in the control group（P〈0.05）.Serum IL-6 and CRP levels and pain scores in the study group were significantly lower in the patients than in the control group（P〈0.05）.Conclusion The treatment of gastrointestinal stromal tumors with 3 D laparoscopic surgery is effective,compared with the traditional laparotomy.The bleeding rate is less during the operation and the pain is reduced,and the patients are quicker and quicker prognosis.
