中药浸泡配合负压封闭引流技术治疗糖尿病足临床研究

Clinical Study of Chinese Medicine Soaking Combined with Negative Pressure Closed Drainage Technique in the Treatment of Diabetic Foot

目的：研究中药浸泡配合负压封闭引流技术（VSD）治疗糖尿病足创面的临床效果及其机制。方法：将Wagner分级处于3~4级的60例糖尿病足患者随机分为2组,治疗组30例采用我科自拟糖尿病足2号浸泡方行中药浸泡配合VSD治疗,对照组30例采用常规换药配合VSD治疗。以2组患者创面愈合率、创面愈合时间、住院时间、住院费用及疗效指标进行对比研究。结果：治疗组创面愈合率明显高于对照组（P〈0.05）,创面愈合时间、住院时间均明显少于对照组（P〈0.05）,住院费用明显低于对照组（P〈0.05）。治疗组治愈率66.7%,对照组40%,2组比较差异显著（P〈0.05）。结论：与传统的常规换药配合VSD治疗比较,中药浸泡配合VSD治疗能更快的促进创面愈合,缩短治疗时间,降低住院费用,提高疗效,是治疗糖尿病足一种切实高效的方法,可供临床推广。

Objective：To study the clinical effect and mechanism of Chinese medicine soaking with negative pressure closed drainage（VSD）in the treatment of diabetic foot wound. Methods：60 patients with diabetic foot who were graded at 3-4 level in Wagner were randomly divided into 2 groups. 30 patients in the treatment group were treated with Chinese medicine self-made diabetic foot 2 soaking recipe,and Chinese medicine soaked with VSD. 30 patients in the control group were treated by regular dressing change combined with VSD. The wound healing rate,wound healing time,hospitalization time,hospitalization expenses and therapeutic index were compared in the 2 groups. Results：The wound healing rate in the treatment group was significantly higher than that in the control group（P〈0.05）,the wound healing time and hospitalization time were significantly less than those in the control group（P〈0.05）,and the cost of hospitalization was significantly lower than that in the control group（P〈0.05）. In the treatment group,the cure rate was 66.7%,the control group was 40%,and the 2 groups were significantly different（P〈0.05）.Conclusion：Compared with traditional VSD dressing,traditional Chinese medicine soaking combined with VSD can accelerate wound healing,shorten treatment time,reduce hospitalization expenses and improve curative effect. It is a practical and effective method to treat diabetic foot,and it can be used for clinical promotion.
