不同方法治疗老年胃癌术中出血量及肛门排气时间等的临床比较

Clinical Comparison of Different Methods for Treatment of Elderly Gastric Blood Loss and Anus Exhaust Time

目的对比分析老年胃癌患者分别采用传统开腹术以及腹腔镜辅助胃癌根治术两种不同方法治疗的术中出血量及肛门排气时间等情况。方法方便选取该院2015年3月—2017年6月收治的58例老年胃癌患者按照不同手术治疗方式采取随机抽签法分为实验组(29例,采用腹腔镜辅助胃癌D2根治术)和对照组(29例,采用传统D2根治术),对比两组患者手术指标及术后并发症发生率。结果实验组患者术中出血量明显少于对照组(P<0.05),实验组患者手术时间、术后肛门排气时间、住院时间等指标均短于对照组(P<0.05)。实验组患者术后出现肺部感染、肠梗阻、切口感染等并发症发生率6.9%显著低于对照组24.1%(χ2=7.293,P<0.05)。结论老年胃癌患者采用腹腔镜辅助胃癌D2根治术治疗效果佳,不仅可有效减少患者术中出血量,降低术后并发症发生率,也可缩短患者术后肛门排气时间、住院时间,更有利于提高患者预后。

Objective To compare the intraoperative blood loss and anal exhaust time between elderly patients with gastric cancer and traditional open surgery and laparoscopic radical gastrectomy. Methods A total of 58 elderly patients with gastric cancer admitted to the hospital from March 2015 to June 2017 were randomly divided into experimental group（29 cases, using laparoscopic assisted gastric cancer D2 radical resection） and control group according to different surgical treatments. In 29 patients, the traditional D2 radical operation was used to compare the surgical indexes and the incidence of postoperative complications between the two groups. Results The amount of intraoperative blood loss in the experimental group was significantly lower than that in the control group（P〈0.05）. The experimental time, operation time, postoperative anus exhaust time, length of hospital stay and other indicators in the experimental group were shorter than those in the control group（P〈0.05）. The incidence of postoperative complications such as pulmonary infection, intestinal obstruction, and wound infection was significantly lower in the experimental group than in the control group（6.9% vs 24.1%）（χ^2=7.293, P〈0.05）. Conclusion The treatment of elderly patients with gastric cancer using laparoscopic assisted gastric cancer D2 radical surgery is effective, not only can effectively reduce the amount of intraoperative blood loss, reduce the incidence of postoperative complications, but also can shorten postoperative patient anus exhaust time, hospital stay, and help to improve patients＇ prognosis.
