摘要
目的评价无托槽隐形矫正治疗Ⅲ类错颌疗效、起效特点,为数字化设计提供依据。方法 2017年2—9月,医院共采用无托槽隐形矫治器矫正治疗Ⅲ类错颌8例,测量184颗牙,对比设计目标以及实际矫治指标。结果颊向、舌向、近中、远中、伸长、压低方向上的移动效能分别为(67.4±25.6)%、(68.4±21.7)%、(62.9±30.4)%、(77.2±41.1)%、(60.4±29.1)%、(56.1±27.5)%,差异有统计学意义(P<0.05)。倾斜矫治效果与近远、近中的矫治效果存在显著相关性,差异有统计学意义(r=0.641、0.681,P<0.05)。结论无托槽隐形矫正治疗Ⅲ类错颌疗效尚可,对于近远中移动需求较高的对象,需要重点提升倾斜方向移动控制。坚持个体化的设计原则,重视倾斜方向移动力矩控制,以增强整体移动能力,最终提高矫治效果。
Objective To evaluate the efficacy and onset characteristics of type III malocclusion without brackets invisible correction, and provide evidence for digital design. Methods From February to September of 2017, the hospital used a nonbracket invisible appliance to correct 8 cases of type III malocclusion and measure 184 teeth, comparing the design goals and the actual correction index. Results The movement efficiencies in the buccal, lingual, mesial, distal, elongate and depressive directions were(67.4±25.6)%,(68.4±21.7)%,(62.9±30.4)%,(77.2±41.1)%,(60.4±29.1)%,(56.1±27.5)% respectively. The difference was statistically significant(P〈0.05). There was a significant correlation between the effect of oblique correction and the correction effect of near-distant and near-mediation, and the difference was statistically significant(r =0.641, 0.681, P〈0.05). Conclusion The treatment effect of Class III malocclusion with no bracket invisible correction is acceptable. For the subjects with high mobile demand in near and far, it is necessary to focus on improving the control of tilt direction. Adhere to the individual design principles, pay attention to the tilt direction movement torque control to enhance the overall mobility, and ultimately improve the effect of correction.
