50岁以下绝经与未绝经女性冠心病患者冠状动脉病变特点和相关因素分析

Characteristics and related factors of coronary artery disease in premenopausal and postmenopausal women under 50 years of age

目的分析50岁以下绝经与未绝经女性冠心病患者冠状动脉病变特点和相关因素。方法选取冠状动脉造影证实为冠心病的50岁以下女性患者154例,其中绝经组46例和未绝经组108例。对比分析2组冠心病患者冠状动脉病变特点和相关因素。结果未绝经组既往有糖尿病者少于绝经组,吸烟和有冠心病家族史者多于绝经组,未绝经组三酰甘油低于绝经组,未绝经组以单支血管病变为主,而绝经组以多支血管病变为主,差异均有统计学意义(P<0.05)。结论冠状动脉疾病在未绝经妇女中并不罕见,发病率增加,但在临床实践中往往被低估,故在临床工作中不能忽视。

Objective To analyze the features of coronary artery disease and releated factors for postmenopausal and premenopausal women aged below 50 with coronary heart disease.Methods One hundred and fifty-four female patients who were aged below 50 and diagnosed with coronary heart disease by coronary arteriography were divided into postmenopausal group（46 women）and premenopausal group（108 women）.The features of coronary artery disease and releated factors for 2 groups of patients were compared and analyzed.Results The age of the menopausal group was higher than that of the non-menopausal group（P〈0.01）.Premenopausal group had less diabetes than the menopausal group,more smoking and more family history of coronary heart disease.Triacylglycerol in the premenopausal group was lower than that in the menopausal group.The premenopausal group was dominated by single-vessel disease,while the menopausal group was dominated by multi-vessel disease,and the difference was statistically significant（P〈0.05）.Conclusion Coronary artery disease is not uncommon in premenopausal women with an increased incidence,but it is often underestimated in clinical practice and cannot be ignored in clinical work.
