低分子肝素钙早期干预防治呼吸衰竭患者凝血功能异常的临床研究

Clinical Study on Early Intervention of Low Molecular Weight Heparin Calium in Preventive Treatment of Dysfunction of Blood Coagulation of Patients with Respiration Failure

目的探讨低分子肝素钙早期干预防治呼吸衰竭患者凝血功能异常的临床效果。方法选取该院2015年1月—2017年1月收治的120例呼吸衰竭患者为研究对象,采用随机数字表法分为两组,观察组60例患者给予低分子肝素钙治疗,对照组60例患者给予常规治疗,对比两组患者临床疗效和治疗前后血气分析指标以及凝血功能指标。结果治疗后观察组患者pH、PaCO_2、PaO_2分别为(7.36±0.12)、(7.2±1.4)kPa、(12.0±1.4)kPa,明显优于对照组(7.15±0.13)、(8.3±1.3)kPa、(9.6±1.5)kPa,差异有统计学意义(t=9.194 4、4.459 9、9.060 4,P<0.05)。治疗后观察组患者APTT、PT、D-dimer、PLT分别为(40.0±5.4)s、(18.5±2.2)s、(1.05±1.19)mg/L、(79.7±14.2)×10~9/L,明显优于对照组(43.3±5.3)s、(16.5±2.1)s、(5.78±1.23)mg/L、(56.7±13.7)×109/L,差异有统计学意义(t=3.378 3、5.093 7、21.408 0、9.029 1,P<0.05)。结论呼吸衰竭患者给予低分子肝素钙治疗可有效提高临床疗效,改善血气指标、凝血功能,值得临床推广。

Objective To study the clinical effect of low molecular weight heparin calium in preventive treatment of dysfunction of blood coagulation of patients with respiration failure. Methods 120 cases of patients with respiration failure admitted and treated in our hospital from January 2015 to January 2017 were selected and divided into two groups with 60 cases in each, the observation group were treated with low molecular weight heparin calium, while the control group used the routine therapy, and the clinical curative effect, blood gas analysis indictors and blood coagulation indicators before and after treatment were compared between the two groups. Results After treatment, the pH, PaCO_2, PaO_2 in the observation group were obviously better than those in the control group [（7.36 ±0.12）,（7.2 ±1.4）kPa,（12.0±1.4）kPa vs（7.15±0.13）,（8.3±1.3）kPa,（9.6±1.5）kPa], and the differences were statistically significant（t=9.194 4,4.459 9, 9.060 4, P〈0.05）, after treatment, the APTT, PT, D-dimer, PLT in the observation group were obviously better than those in the control group[（40.0±5.4）s,（18.5±2.2）s,（1.05±1.19）mg/L,（79.7±14.2）×109/L vs（43.3±5.3）s,（16.5±2.1）s,（5.78±1.23）mg/L,（56.7±13.7）×10^9/L], and the differences were obvious, with statistical significance（t=3.378 3, 5.093 7,21.408 0, 9.029 1, P〈0.05）. Conclusion The low molecular weight heparin calium in preventive treatment of dysfunction of blood coagulation of patients with respiration failure can effectively improve the clinical curative effect, improve the blood index and coagulation function, which is worth clinical promotion.
