摘要
目的对经腹、经阴道超声监测不孕症卵泡的临床效果作对比。方法选入研究对象为该院2016年1月—2017年12月接收的不孕症患者100例,为其均行经腹部、经阴道超声检查;将两种超声诊断方法对不孕症卵泡监测的临床价值作对比分析。结果经腹部超声监测提示卵巢142个、卵丘4个、小卵泡219个、成熟卵泡63个;经阴道超声监测提示卵巢167个、卵丘9个、小卵泡391个、成熟卵泡92个,不孕症患者经阴道超声监测小卵泡、卵丘数量比经腹部多(χ~2=4.904 5、26.273 1,P<0.05)。经腹部超声诊断结果见患者子宫内膜厚度为(8.14±1.50)mm、卵泡排出直径为(20.47±2.43)mm;经阴道超声诊断结果见患者子宫内膜厚度为(8.20±1.64)mm、卵泡排出直径为(20.81±2.56)mm,两种方法监测不孕症患者子宫内膜及卵泡直径结果差异无统计学意义(t=7.654 1、19.718 3,P>0.05)。超声检查时可见卵巢中具有小卵泡,并见其逐渐发育长大,第14天时卵泡直径最大并排出,不同周期卵泡图像特点均是不同的。结论不孕症患者接受经阴道超声检查卵泡的清晰度、准确度均高,但两种方式均有其局限性,临床上使用时可将其联合应用。
Objective To compare the effect of transabdominal and transvaginal ultrasound monitoring of infertile follicles. Methods 100 cases of patients with infertility admitted and treated in our hospital from January 2016 to December 2017 were selected, for transabdominal and transvaginal ultrasound monitoring, and the clinical value of the two diagnosis methods in monitoring of infertile follicles was compared and analyzed. Results The transabdominal monitoring reminded that there were 142 ovaries, 4 ovarian cumulus, 219 small ovarian follicles, and 63 matured follicles; the transvaginal ultrasound monitoring remineded that there were 167 ovaries, 9 ovarian cumulus, 391 small ovarian follicles, and 92 matured follicles, and the number of small follicle and cumulus monitored by the transvaginal ultrasound was more than that of transabdominal monitoring(χ^2=4.904 5, 26.273 1,P〈0.05). The transabdominal diagnosis and transvaginal results showed that the endometrium thickness and follicle discharge diameter were respectively(8.14±1.50) mm,(20.47±2.43) mm and( 8.20±1.64) mm,( 20.81±2.56) mm, and the differences were not obvious(t=7.654 1,19.718 3,P〈0.05), and the ultrasound examination showed that the small follicles were in the ovary, and gradually developed,and the diameter of follicles at 14 d was the biggest, and they were discharged, and the features of follicle images at different cycles were different. Conclusion The clearness and accuracy of transvaginal ultrasound monitoring of infertile follicles are high, but both have limitations, and we had better combine the two methods in clinic.
