摘要
目的探讨高场强磁共振(MRI)检查在胆道系统结石诊断中的应用价值。方法选取2016年1月—2017年5月于该院经外科手术证实的56例胆道系统结石患者作为研究对象。所有患者均具有完整的MRI、CT、超声检查结果,对比3种影像学检查结果诊断胆道系统结石的结果,并与手术证实结果进行比较。结果 MRI检查胆道系统结石确诊率为92.86%,明显高于CT确诊率75.00%与超声确诊率71.43%(P<0.05),但CT与超声检查确诊率比较差异无统计学意义(P>0.05);在不同直径结石诊断中,D≥0.8 cm的结石确诊率中,MRI为100.00%,CT确诊率为92.00%,超声为88.00%,3种检查方案在D≥0.8 cm的结石确诊率比较中差异无统计学意义(P>0.05),但在D<0.8 cm结石诊断中MRI确诊率为83.87%,CT确诊率为61.29%,超声确诊率为58.06%,MRI确诊率明显高于CT、超声(P<0.05),且CT与超声确诊率比较差异无统计学意义(P>0.05)。结论采用高场强MRI检查胆道系统结石,其确诊率明显高于CT及超声,尤其是对于直径小于0.8 cm的结石具有明显的诊断优势,具有临床应用及推广价值。
Objective To study the high-field MRI in diagnosis of stones of biliary tract system and its application value.Methods 56 cases of patients with stones of biliary tract system proved by the surgical surgery in our hospital from January 2016 to May 2017 were selected,all patients had the complete MRI,CT and ultrasound examination results,and the results of three imaging examination and diagnosis of stones of biliary tract system were compared,with the operation.Results The confirmation rate of MRI in diagnosis of stones of biliary tract system was obviously higher than that of CT and ultrasound,(92.86% vs 75.00% vs 71.43%)(P〈0.05),but the difference in the confirmation rate between the CT and ultrasound was not obvious(P〉0.05),in terms of diagnosis of different diameters of stones D≥0.8 cm,the confirmation rate of MRI,CT and ultrasound was respectively 100.00%,92.00%,88.00%,and the difference was not obvious(P〉0.05),but confirmation rate of stones D0.8 cm was respectively 83.87%,61.29%,58.06%,and the MRI confirmation rate was obviously higher than that of CT and ultrasound(P〈0.05),but the difference in the confirmation rate between CT and ultrasound was not obvious(P〉0.05).Conclusion The confirmation rate of high-field MRI in diagnosis of stones of biliary tract system is obviously higher than that of CT and ultrasound,especially for stones D 0.8 cm,it has an obvious diagnosis advantage,and it is of clinical application and promotion value.
