摘要
目的探讨在不同分子生物学亚型乳腺癌哺乳动物雷帕霉素靶蛋白(mTOR)通路中的表皮生长因子受体(EGFR)过表达与其下游分子4E结合蛋白1(4EBPl)表达异常的关系和临床意义。方法应用免疫组织化学方法检测111例浸润性乳腺癌组织中EGFR和4EBPl的表达,分析在不同分子生物学亚型中两者的表达相关性。同时分析两者的表达的相关性及与淋巴结转移,肿瘤TNM分期以及5年无病生存期(DFS)的关系。结果EGFR和4EBPl在不同分型乳腺癌中的阳性表达结果显示两者均在三阴型乳腺癌中表达较高,分别为21例(43.8%)、20例(41.7%),其中EGFR表达差异有统计学意义(X^2=7.000,P=0.030),而4EBPl表达差异无统计学意义(X^2=1.722,P=0.423)。EGFR与4EBPl的过表达Spearman相关分析结果显示,两者的表达在α=0.01(r=0.453,P=0.000)水平显著相关。EGFR与4EBPl的高表达均与淋巴结转移(r=0.520,P=0.000;r=0.444,P=0.000)在和肿瘤TNM分期(r=0.533,P=0.000;r=0.273,P=0.004)在α=0.01水平显著相关。结论4EBPl的表达可能受EGFR的调控。EGFR和4EBPl的高表达对于乳腺癌的腋窝淋巴结转移有促进作用。
Objective To explore the correlation between over - expression of epidermal growth factor receptor (EGFR) and aberrant expression of p -4E binding protein 1 (4EBP1) in different biologi- cal subtype breast carcinoma. Methods 111 cases of invasive breast carcinoma were included. Immuneo- histochemistry was applied to analyze the expression of EGFR and 4EBP1 in all cases. The correlation be- tween EGFR and 4EBP1 expression in different biological subtype breast carcinoma was analyzed, and sim- ultaneously their correlations with lymphatic metastasis, TNM neoplasm staging and the five -year disease free survival were discussed. Results The positive expression rate of EGFR and 4EBP1 in triple - nega- tive breast cancer was 43.8% (X2 =7. 000, P =0. 030) and 41.7% (X2 = 1. 722, P =0. 423) respective- ly. There was significant difference in the EGFR expression ( r = 0. 453, P = 0. 000). Spearman regres- sion analysis revealed that there was a close correlation between EGFR and 4EBP1 over - expression. The overexpression of both EGFR and 4EBP1 was correlated with lymph node metastasis ( r = 0. 520, P = 0. 000; r =0.444, P=0.000) and TNM staging (r =0.533, P= 0.000; r =0.273, P=0.004). There were no statistical differences in disease -free survival. Conclusion The expression of 4EBP1 was possi- bly regulated bv EGFR. Their expression mav promote the process of lymphatic metastasis.
