低血糖恐惧量表-15在2型糖尿病患者中的信效度检验

Translation and psychometric evaluation of a new Fear of Hypoglycemia Scale (FH-15) in type 2 diabetes patients

目的汉化低血糖恐惧量表-15(FH-15)并在2型糖尿病患者中检验信度和效度。方法对英文版FH-15量表进行翻译、回译和文化调适,形成中文版FH-15;抽取天津市4所三级甲等医院408例2型糖尿病患者进行调查,检验其信效度。结果量表内容效度指数为0.920;探索性因子分析提取3个公因子(恐惧、回避、干扰),累积方差贡献率为71.245%;验证性因子分析显示模型拟合度较好。低血糖恐惧感的截断值为30.5,此时AUG为0.816,敏感度为0.748,特异度为0.835。量表Cronbach's α系数0.918,各维度Cronbach's α系数0.876~0.916,重测信度0.903;与低血糖恐惧调查表-忧虑子量表的效标效度r为0.578。结论中文版FH-15量表具有良好的信效度并确定截断值,可用于2型糖尿病患者低血糖恐惧筛查。

Objective To translate a new Fear of Hypoglycemia Scale（FH-15）into simplified Chinese and evaluate the reliability and validity in type 2 diabetes patients.Methods Translation,back-translation,and cultural adaptation were used to develop the Chinese version of FH-15.Then 408 type 2 diabetes patients were selected from 4 tertiary hospitals in Tianjin and were investigated to validate the scale.Results Content validity index of the scale was 0.920.Three factors（fear,avoidance,and interference）were identified using exploratory factor analysis,accounting for 71.245% of the total variance.Confirmatory factor analysis indicated acceptable model fit.The optimal cut-off value for hypoglycemia fear was 30.5,with the AUG of 0.816,sensitivity of 0.748,and specificity of 0.835.The Cronbach’s α coefficient for the total scale was 0.918,and for three dimensions ranged from 0.876 to 0.916.The test-retest reliability was 0.903,and the criterion validity using the Hypoglycemia Fear Survey-Ⅱ worry subscale was 0.578.Conclusion The Chinese version of the FH-15 has good reliability and validity.The cut-off value is also determined,which can be used to identify hypoglycemia fear of patients with type 2 diabetes.
