静脉血管扩张剂在急性失代偿心力衰竭中的应用进展

Application of Intravenous Vasodilator in Acute Decompensated Heart Failure

随着治疗药物、器械植入以及外科手术治疗理念的不断进展,各种急性心血管疾病的长期存活率得到显著提高。相当数量的存活患者将逐步进展至慢性心力衰竭,并不断经历各种原因导致的心力衰竭的急性失代偿。与慢性心力衰竭治疗取得了实质性进展不同,急性失代偿心力衰竭的基础理论与药物治疗策略均有待进一步的深入研究与评估。在急性失代偿心力衰竭治疗的临床实践中,血管扩张剂是继利尿剂后的另一种重要的基石性药物,但其应用目前尚存诸多争议,缺乏充分的证据。现旨在综述急性失代偿心力衰竭患者使用静脉血管扩张剂的病理生理机制、相关临床循证试验证据、适应证优化以及可能存在的风险。

As the medical technology developing,the survival rate of all acute cardiovascular disease is going up.While most of these patients will develop into heart failure,and suffer from acute decompensated heart failure due to various reasons.The treatment of acute decompensated heart failure is still a puzzle to us.At the time now,the treatment of acute decompensated heart failure is still based on the experience of physican,especially the using of vasodilator.There is also debate in using of vasodilator,as a result of lacking of evidence.It is very important to know about the pathophysiological process,appropriate indication and the corresponding risk by using of vasodilator.
