血管外基质蛋白Fibulin-1在早期糖尿病肾病中的表达及临床意义

The expression and clinical significance of extravascular stromal protein fibulin-1 in early diabetic nephropathy

目的探究血管外基质蛋白Fibulin-1在早期糖尿病肾病(DN)中的表达情况及其临床意义。方法选取兴化市人民医院2017年2-6月住院患者90例,根据晨尿微量清蛋白水平,将其分为糖尿病组30例(尿微量清蛋白0~30mg/L),糖尿病微量清蛋白尿组30例(>30~300mg/L),糖尿病大量蛋白尿组30例(>300mg/L);另选体检健康者30例为健康对照组,分析4组血清中胱抑素C(Cys C)、空腹血糖(FBG)、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)和血清中Fibulin-1表达。结果糖尿病大量蛋白尿组Cys C水平和健康对照组以及糖尿病组之间差异有统计学意义(P<0.05)。糖尿病大量蛋白尿组的Fibulin-1的水平与健康对照组、糖尿病组及糖尿病微量蛋白尿组之间差异有统计学意义(P<0.05),糖尿病微量蛋白尿组与健康对照组及糖尿病组之间差异有统计学意义(P<0.05)。采用Spearman相关分析发现,血清Fibulin-1水平与血清Cys C、FBG、TG、TC、LDL-C均呈正相关(r分别为0.46、0.38、0.29、0.37、0.35,P<0.05)。结论血清Fibulin-1水平与DN的发生及病情严重程度相关。检测血清Fibulin-1水平对DN的病情判断具有积极的临床意义。

Objective To investigate the expression of extracellular matrix protein Fibulin-1 in early diabetic nephropathy and its clinical significance. Methods A total of 90 cases of hospitalized patients in the hospital from February to June 2017 were divided into 30 cases of diabetic group （urine trace albumin content 0-30 mg/L）,30 cases of diabetes microalbuminuria group （〉30-300 mg/L）,30 cases of diabetes albuminuria group （〉300 mg/L）,30 healthy subjects were selected as the healthy control group.The serum levels of cystatin C （Cys C）,fasting blood glucose （FBG）,three acyl glycerol （TG）,total cholesterol （TC）,low density lipoprotein cholesterol （LDL-C）,high density lipoprotein cholesterol （HDL-C） and serum Fibulin-1 expression in 4 groups were analyzed. Results There was a significant difference in the level of Cys C between the diabetes albuminuria group and the healthy control group and the diabetes group （ P 〈0.05）.The levels of Fibulin-1 in the diabetes albuminuria group were significantly different from those in the healthy control group,the diabetic group and the diabetic microalbuminuria group （ P 〈0.05）.There was significant difference between diabetic microalbuminuria group and healthy control group and diabetic group （ P 〈0.05）.Spearman correlation analysis showed that serum Fibulin-1 level was positively correlated with serum Cys C,FBG,TG,TC and LDL-C （ r =0.46,0.38,0.29,0.37,0.35, P 〈0.05,respectively）. Conclusion Serum Fibulin-1 level is associated with the occurrence and severity of diabetic nephropathy.Detection of serum Fibulin-1 level has a positive clinical significance in the diagnosis of diabetic nephropathy.
