穿刺粉碎术在老年脑出血患者中的应用效果

Application effect of puncture comminution surgery in elderly patients with intracerebral hemorrjage

目的探究穿刺粉碎术在老年脑出血(ICH)患者中的应用效果。方法选取本院收治的高血压ICH老年患者92例作为研究对象,并分为传统组和微创组,每组46例。传统组给予开颅清除术,微创组给予微创穿刺粉碎术,观察两组患者临床治疗效果。结果治疗后,两组患者基线血肿体积和GCS评分均较治疗前有所好转,微创组GCS评分低于传统组(P<0.05);微创组NIHSS、GOS及Barthel评分均优于传统组(P<0.05);微创组临床一般情况优于传统组(P<0.05);微创组并发症总发生率低于传统组(P<0.05)。结论YL-1型微创穿刺粉碎针可有效消除高血压ICH患者颅内血肿,缩短患者住院时间、手术时间,降低手术费用。

Objective To observe the application effect of puncture comminution surgery in elderly patients withintracerebral hemorrjage （ICH）. Methods A total of 92 elderly patients with hypertensive ICH treated in our hospital wereselected as research objects. The patients were divided into conventional group and minimally invasive group, with 46 casesin each group. The conventional group was given craniotomy clearance surgery, the minimally invasive group was givenminimally puncture comminution surgery. The clinical treatment effects of the two groups were observed. Results Aftertreatment, the baseline hematoma volume and GCS scores in the two groups significantly improved, and GCS score in theminimally invasive group was better than that in the conventional group （P〈0.05）. The NIHSS score, GOS score andBarthel score in the minimally invasive group were better than those in the conventional group （P〈0.05）. The generalclinical situation in the minimally invasive group were better than those in the conventional group （P〈0.05）. The totalincidence of complications in the minimally invasive group was lower than that in the conventional group （P〈0.05）.Conclusion The puncture comminution surgery in elderly patients with hypertensive ICH can effectively relieve theintracranial hematoma, shorten the hospitalization time, operation time and reduce the hospitalization costs.
