79576例行宫颈脱落细胞学检查患者中阳性病例的病理诊断及年龄特征

Pathological diagnosis and age characteristics of positive cases in 79576 patients undergoing cervical exfoliative cytolgical examination

目的分析79 576例宫颈脱落细胞学检查患者中阳性病例的年龄及相关的临床诊断特征,提高对宫颈脱落细胞学在筛查宫颈癌及癌前病变中重要价值的认识。方法分析79 576例行宫颈脱落细胞学检查的患者中阳性病例的年龄和病理诊断结果。结果 79 576例宫颈脱落细胞学检查的患者中,阳性病例870例,占比为1.09%。在870例阳性病例的检查结果中,ASC-US占61.26%,ASC-H占15.98%,LSIL占5.52%,HSIL占8.62%,AGC占0.69%。各年龄段均有宫颈细胞学异常出现,其中36~40岁的患者最多。阳性病例的临床表现有宫颈炎、下腹痛、接触性出血、白带夹血丝、阴道分泌物增多及阴道排液等。结论宫颈癌前病变的发生人群越来越趋向年轻化,且其临床表现呈现多样化,因此应加强对宫颈脱落细胞学诊断的认识,并将其科普推广应用于宫颈癌筛查,进而提高宫颈癌及宫颈癌前病变的诊断率。

Objective To analyze the age and related clinical diagnostic characteristics of positive cases in 79 576patients undergoing cervical exfoliative cytolgical examination and improve the awareness of the cervical exfoliativecytolgical examination value in screening for cervical cancer and precancerous lesions. Methods The age and pathologicaldiagnosis results of positive cases in 79 576 patients undergoing cervical exfoliative cytolgical examination were analyzed.Results Among 79 576 patients undergoing cervical exfoliative cytolgical examination, 870 cases were positive, accountingfor 1.09%, and including ASC-US accounting for 61.26%, ASC-H accounting for 15.98%, LSIL accounting for 5.52%,HSIL accounting for 8.62% and AGC accounting for 0.69%. Cervical cytologic abnormalities were found in all age groups,of which 36 -40 years of age were the most common. The clinical manifestations of the positive cases were cervicitis,hypogastralgia, contact bleeding, leucorrhea with blood filaments, increasing vaginal fluid, vaginal discharge and so on.Conclusion The morbidity crowd of cervical precancerous lesions tends to be younger and there are various clinicalmanifestations. Therefore, we should strengthen the understanding of cervical exfoliative cytolgical examination, andpopularize the application of it in screening for cervical cancer, so as to improve the diagnosis rate of cervical cancer andcervical precancerous lesions.
