湿咳方治疗儿童肺炎支原体肺炎疗效及对肺功能、CRP的影响 被引量:1

Effect of wet cough recipe on mycoplasma pneumoniae pneumonia in children and its influence on pulmonary function and CRP

摘要 目的观察湿咳方治疗肺炎支原体肺炎疗效,探讨其对肺功能、CRP的影响。方法选择我院2017年1~12月收治的140例符合入组标准的儿童肺炎支原体肺炎作为研究对象,按随机数字表法分为观察组70例和对照组70例,对照组给予阿奇霉素序贯治疗,观察组在对照组的基础上再口服湿咳方治疗,两组均治疗2周,记录两组患儿退热时间、咳嗽消失时间、咯痰消失时间、肺部啰音消失时间,治疗前后进行肺功能指标[潮气量(V-T)、达峰时间比(t-PTEF/t-E)、呼出75%潮气量时间与潮气呼气流速比值(TEF25/PTEF)]、CRP检测,治疗2周后进行临床疗效评价,记录不良反应发生情况。结果观察组和对照组的退热时间分别为(3.2±1.3)d vs(5.5±1.6)d,咳嗽消失时间分别为(4.0±1.6)d vs(7.2±1.8)d,咯痰消失时间分别为(3.6±1.4)d vs(7.0±1.5)d,肺部啰音消失时间分别为(5.2±2.6)d vs(8.0±2.8)d,差异有统计学意义(均P <0.05)。观察组和对照组治疗后血清CRP均较治疗前明显下降(P <0.05),V-T、t-PTEF/t-E、TEF25/PTEF较治疗前明显升高(P <0.05),观察组治疗后CRP明显低于对照组(P <0.05),V-T、t-PTEF/t-E、TEF25/PTEF明显高于对照组(P <0.05)。观察组和对照组治疗2周的临床总有效率分别为94.29%vs 80.00%,差异有统计学意义(P <0.05)。观察组和对照组不良反应发生率分别为7.14%vs 20%,差异有统计学意义(P <0.05)。结论阿奇霉素序贯疗法治疗儿童肺炎支原体肺炎的基础上加用湿咳方能够缩短症状、体征消失时间,改善肺功能,提高临床治疗效果。 Objective To observe the curative effect of wet cough recipe on mycoplasma pneumoniae pneumonia and to explore its effect on pulmonary function and CRP. Methods A total of 140 children with mycoplasma pneumoniae pneumonia who met the admission criteria in our hospital from January to December 2017 were selected as the research subjects.According to the random number table,there were 70 cases in the observation group and 70 cases in the control group.The control group was given Azithromycin Sequential treatment,and the observation group was given wet cough Recipe on the basis of the control group.Both groups were treated for 2 weeks.The fever reduction time,cough disappearance time,sputum expectoration disappearance time and lung rales disappearance time were recorded in the two groups.Before and after treatment,lung function index [tidal volume (V-T),peak time ratio (t-PTEF/t-E),exhaled 75% tidal volume time to tidal expiratory flow rate ratio (TEF25/PTEF)] and CRP were detected. The clinical efficacy was evaluated after 2 weeks of treatment.The occurrence of adverse reactions was recorded. Results The defervescence time of the observation group and the control group were (3.2 ± 1.3)d vs (5.5 ±1.6) d,cough disappearance time was (4.0 ± 1.6)d vs (7.2 ± 1.8)d,expectoration disappearance time was (3.6± 1.4)d vs (7.0 ± 1.5)d,lung rale disappearance time was (5.2 ±2.6)d vs (8.0± 2.8)d,respectively,the difference was statistically significant (all P 〈 0.05).After treatment,serum CRP in observation group and control group decreased significantly (P 〈 0.05),V-T,t-PTEF/t-E,TEF25/PTEF increased significantly (P 〈 0.05).After treatment,CRP in the observation group was significantly lower than that in the control group (P 〈 0.05),V-T,t-PTEF/t-E,TEF25/PTEF were significantly higher than that in the control group (P 〈 0.05).The total clinical effective rate of the observation group and the control group was 94.29% vs 80.00% after 2 weeks of treatment,the difference was statistically significant (P 〈 0.05).The incidence of adverse reactions in the observation group and the control group was 7.14% vs 20%,the difference was statistically significant (P 〈 0.05). Conclusion On the basis of azithromyein sequential therapy for myeoplasma pneumoniae pneumonia in children,the addition of wet cough formula can shorten the time for symptoms and signs to disappear,improve lung function and improve the clinical treatment effect.
