踝足矫形器在小儿脑瘫康复中的应用价值分析

Application value of ankle foot orthosis in rehabilitation of cerebral palsy in children

目的研究并探讨踝足矫形器在小儿脑瘫康复中的应用价值。方法选择2015年1月~2017年6月期间我院收治的100例小儿脑瘫患儿,在计算机中按照随机数字表法随机分为两组,各50例,对照组患儿接受常规康复训练,观察组患儿在常规康复训练基础上佩戴踝足矫形器,随访观察6个月,比较两组脑瘫患儿的康复效果、智力评分、运动功能评分、独立性评分、肌力改善情效果、步态情况。结果在康复效果方面,观察组患儿的总有效率为86%,对照组为68%,组间比较差异有统计学意义(P <0.05)。护理后,观察组智力评分、运动功能评分、独立性评分均高于对照组(P <0.05)。观察组肌力改善有效率为92%,对照组为76%,组间比较差异有统计学意义(P <0.05)。护理后,观察组患儿的10m最快步行速度高于对照组(P <0.05),其跨步长长于对照组(P <0.05),其患侧-健侧步长差值小于对照组(P <0.05)。结论在小儿脑瘫患儿康复训练中应用踝足矫形器佩戴,可有效提高康复训练效果,更加有效地改善患儿肌力,对其智力、运动功能、独立性、步态均有改善作用。

Objective To study and discuss the application value of ankle-foot orthosis in the rehabilitation of children with cerebral palsy. Methods One hundred children with cerebral palsy treated in our hospital from January 2015 to June 2017 were selected.According to the random digital table method,the patients in the computer were randomly divided into two groups：50 eases in each group.The children in the control group received routine rehabilitation training, the children in the observation group wore ankle foot orthosis on the basis of routine rehabilitation training,and were followed up for 6 months.The rehabilitation effect,intelligence score,motor function score,independence score,muscle strength improvement effect and gait were compared between the two groups, Results In terms of rehabilitation effect, the total effeetive rate of the observation group was 86% and that of the eontrol group was 68%. There was signifieant differenee between the two groups（P 〈 0.05）.After nursing, the scores of intelligence, motor function and independence in observation group were higher than those in control group （ P 〈 0.05）.The effective rate of muscle strength improvement in observation group was 92%,and that in control group was 76%（P 〈 0.05）.The fastest walking speed of 10m in the observation group was higher than that in the eontrol group （P 〈 0.05）,its stride length was longer than that in the eontrol group （P 〈 0.05）,and the difference between the diseased side and the healthy side was smaller than that in the control group （ P 〈 0.05）. Conclusion The application of ankle and foot orthosis in rehabilitation training of children with cerebral palsy can effectively improve the effect of rehabilitation training,improve the muscle strength of children and improve their intelligenee, motor funetion,independenee and gait.
