普外科疾病采用腹腔镜手术治疗的效果研究

Effect of laparoscopic surgery on general surgical diseases

目的探讨腹腔镜手术治疗普外科阑尾炎疾病的临床效果。方法选取2017年1～12月我院普外科收治的140例阑尾炎患者,按照治疗方式不同将其分为开腹组(70例)和微创组(70例),开腹组患者采取传统开腹手术治疗,微创组患者采取腹腔镜下阑尾炎切除手术,并比较不同组患者手术效果和切口感染情况。结果微创组患者手术时间明显高于开腹组(P <0.05),而微创组患者手术中出血量、胃肠功能恢复时间和排气时间等均明显低于开腹组(P <0.05);微创组患者切口感染率明显低于开腹组(P <0.05);治疗前,微创组和开腹组患者白细胞(WBC)、中性粒细胞(N)和淋巴细胞(L)计数相近差异无统计学意义(P> 0.05),治疗后,微创组患者WBC、N和L计数均明显优于开腹组(P <0.05)。结论与传统开腹手术相比,腹腔镜下阑尾炎切除术具有微创、术后恢复快和切口感染率低等特点,其值得推广借鉴。

Objective To investigate the clinical effect of laparoseopie surgery in the treatment of general surgery appendicitis. Methods From January 2017 to December 2017, one hundred and forty patients with appendicitis who treated in general surgery of our hospital were divided into open group（70 eases） and minimally invasive group（70 eases） according to different treatment methods. The patients in open group were treated with traditional open surgery, the patients in minimally invasive group were treated with laparoseopie appendicitis resection surgery, and the effects and incision infection of different groups were compared. Results The operation time of minimally invasive group was significantly higher than that of open group（P 〈 0.05）, while the volume of bleeding, the recovery time of gastrointestinal function and exhaust time of minimally invasive group were significantly lower than those of open group（P 〈 0.05）. The incision infection rate of minimally invasive group was significantly lower than that of open group（P 〈 0.05）. Before treatment, there was no signifieant differenee in WBC, neutrophil（N） and lymphoeyte（L） counts between the minimally invasive group and the open group（P 〉 0.05）. After treatment, WBC, N and L counts of the minimally invasive group were significantly higher than those of the open group（P 〈 0.05）. Conclusion Compared with the traditional laparotomy, laparoseopie appendectomy has the characteristics of minimally invasive, rapid postoperative recovery and low incision infection rate, which is worth popularizing.
