调强放疗和三维适形放疗在局部晚期非小细胞肺癌患者中应用价值比较

Application Value Comparison of Intensity-modulated Radiotherapy and Three-dimensional Conformal Radiotherapy in the Patients with Locally Advanced Non-small Cell Lung Cancer

目的比较调强放疗和三维适形放疗在局部晚期非小细胞肺癌患者中应用价值。方法选取经病理学确诊的60例局部晚期非小细胞肺癌作为研究对象,采取随机数字表法分为观察组与对照组,对照组30例患者接受三维适形放疗治疗,观察组30例患者接受调强放疗治疗,比较2组患者临床治疗效果、靶区平均剂量、不均匀性指数、适形指数、患者生存时间及1 a生存率。结果观察组与对照组临床治疗总疾病控制率分别为80. 00%、76. 67%,差异无统计学意义(P> 0. 05); 2组患者靶区平均剂量比较差异无统计学意义(P> 0. 05); 2组患者不均匀性指数及适形指数比较差异均有统计学意义(P <0. 05); 2组患者生存时间、1 a生存率比较差异均无统计学意义(P> 0. 05)。结论调强放疗和三维适形放疗在局部晚期非小细胞肺癌患者中应用价值均较高,治疗效果均良好,而调强放疗可降低患者正常器官照射剂量,安全性更高,值得推广应用。

Objective To compare the value of intensity-modulated radiotherapy and three-dimensional conformal radiotherapy in patients with locally advanced non-small cell lung cancer. Methods Sixty- patients wih locally advanced non-small cell lung cancer were divided into two groups. The 30 patients in the control group received the three-dimensional conformal radiation therapy, the 30 patients in the observation group received the intensity- modulated radiation therapy. The clinical effect, the mean dose of target, the nonuniformity index, conformal index, the average survival period and the 1-year survival rate were compared between the two groups. Results The total disease control rate of the observation group （80.00%） was the same as that of the control group （76.67% , P 〉 0.05 ）. There was no difference in the mean dose of target between the two groups （ P 〉 0.05）. There were the statistical differences in the inhomogeneity index and eolfformability index between the two groups （ P 〈 0.05 ）. There was no difference in the survival period and 1-year survival rate between the two groups （P 〉 0. 05 ）. Conclusion Intensity-modulated radiation therapy and three-dimensional conformal radiotherapy are effective in the patients with locally advanced non-small cell lung cancer, intensity-modulated radiation therapy can reduce the exposure dose in patients with normal organs, and has the better security.
