血清β-HCG和孕酮血清水平在宫外孕早期诊断中的意义

Significance of serum β-HCG and progesterone serum levels in early diagnosis of ectopic pregnancy

目的:通过ROC曲线分析β-人绒毛膜促性腺激素(β-HCG)和孕酮对宫外孕的诊断价值及相关临界值确定,增加临床可操作性。方法:抽取2016年1月—2017年12月我院诊治的128例孕妇作为研究对象,根据是否患有宫外孕将孕妇分为两个组,对照组64例为正常孕妇,观察组64例为宫外孕患者。比较两组患者血清β-HCG和孕酮水平,利用ROC曲线分析血清β-HCG和孕酮对宫外孕诊断准确性和临界值。结果:(1)两组间血清β-HCG和孕酮水平存在显著性差异(P<0.05),对照组β-HCG和孕酮水平明显高于观察组(P<0.05)。(2)β-HCG曲线下面积为0.914,而孕酮曲线下面积为0.835,两个指标曲线下面积均有意义(P<0.05),β-HCG临界值为762 IU/L,敏感率为85.00%,特异率为80.00%。孕酮临界值为8.15 ng/mL,敏感率为60.00%,特异率为92.30%。结论:宫外孕患者血清β-HCG和孕酮水平明显低于正常孕妇,两个指标对宫外孕鉴别有良好诊断准确性。

Objective： To analyze the diagnostic value and related critical value of β-human chorionic gonadotropin （β-HCG） and progesterone for ectopic pregnancy by ROC curve, and to increase clinical operability. Methods： A total of 128 pregnant women who were treated in our hospital from January 2016 to December 2017 were enrolled.According to whether or not they had ectopic pregnancy, the pregnant women were divided into two groups, 64 normal pregnant women in the control group and 64 ectopic pregnancy patients in the observation group.The β-HCG and progesterone levels were compared between the two groups. The ROC curve was used to analyze the diagnostic accuracy and critical value of β- HCG and progesterone for ectopic pregnancy. Results：（1）There were significant differences in β-HCG and progesterone levels between the two groups （P〈0.05）. The levels of β-HCG and progesterone in the control group were significantly high- er than those in the observation group （P〈0.05）. （2）The area under the β-HCG curve was 0.914, while the area under the progesterone curve was 0.835. The area under the two index curves was significant （P〈0.05）, the β-HCG threshold was 762 IU/L, and the sensitivity was 85.00. %, the specific rate is 80.00%.（3）The progesterone threshold was 8.15 ng/mL, the sen- sitivity rate was 60.00%, and the specific rate was 92.30%. Conclusion： The levels of β-HCG and progesterone in patients with ectopic pregnancy are significantly lower than those in normal pregnant women. At the same time, the two indicators have good diagnostic accuracy for the identification of ectopic pregnancy.
