摘要
目的本研究对本院超声弹性成像初诊后确认的乳腺恶性肿块患者感染病原菌分布及耐药性进行分析,为临床治疗提供参考。方法回顾性分析本院2012年4月至2016年4月期间诊治的170例超声弹性成像技术诊断的乳腺恶性肿块患者临床资料,BD公司全自动微生物鉴定仪(PhoenixTM)分析病原菌类型,采用药敏试验进行病原菌耐药性分析,酶联免疫吸附测定法分析患者体内雌激素受体和孕激素受体水平。结果超声弹性成像技术确定的乳腺恶性肿块患者中共分离出病原菌170株,其中:革兰阳性菌79株,占46.5%;革兰阴性菌91株,占53.5%。分离的病原菌对抗菌药物均具有较强的耐药性,但革兰阳性菌对万古霉素和林可霉素较敏感,革兰阴性菌均对亚胺培南和头孢西林较敏感;感染患者血清中雌激素受体和孕激素受体水平明显高于对照组和未感染组受试者,差异具有统计学意义(P<0.05)。结论超声弹性成像技术鉴别乳腺恶性肿块患者感染病原菌以革兰阴性菌为主且均具有较强的耐药性,感染患者血清中雌激素受体和孕激素受体水平明显高于对照组和未感染组受试者。
Objective To explore the pathogen distribution and drug resistance in female patients with malignant breast lumps detected by using ultrasound elasticity imaging technique.Methods One hundred and seventy case of female breast lumps detected by using ultrasound elasticity imaging technique in our hospital from Apr 2012 to Apr 2016 were enrolled.The pathogens were identified by using PhoenixTMand the serum levels of PR and FR were detected with ELISA.Results One hundred and seventy strains of pathogens were isolated from the patients,including 79 Gram positive strains and 91 Gram negative strains.All the isolates showed strong drug resistance.The serum levels of PR and FR were significantly higher in the patients with infections than in those without and in healthy controls(Ps〈0.05).Conclusion Patients with malignant masses detected with ultrasound elasticity imaging technique were prone to pathogen infections,which may be related with the increased PR and FR levels.
