Protective effects of DPP-4 inhibitor on diabetic nephropathy and its mechanism

摘要 糖尿病肾病是糖尿病最严重的并发症之一,是致死、致残的重要原因,其防治仍是当今的一大难题。二肽基肽酶(DPP-4)抑制剂通过抑制胰高血糖素样肽1的降解,可改善血糖水平。新近研究提示DPP-4抑制剂有一定的肾脏保护作用如降低尿蛋白、保护足细胞等,其机制包括降低血压、改善脂代谢、抗氧化、减轻炎症反应、抗纤维化、保护血管内皮细胞及抑制糖基化终末产物等。 Diabetic nephropathy(DN), which is one of the most serious complications of diabetes, is an important cause of death and disability. Until now, prevention and treatment of DN is still a difficult problem.Dipeptidyl-peptidase 4(DPP-4) inhibitors improve hyperglycemia by inhibiting the degradation of glucagon like peptide-1 in vivo. Recent researches suggest that DPP-4 inhibitors have renal-protective effects beyond its hypoglycemic effect, such as reducing urine protein and protecting podocytes. The mechanism involve in lowering blood pressure, improving lipid metabolism, anti-oxidation, anti-inflammatory and anti-fibrosis,protecting renal vascular endothelial cells and inhibition of glycation end products.
