卡维地洛联合依那普利治疗老年风湿性心脏病合并慢性心力衰竭的临床效果观察

Clinical effect of carvedilol combined with enalapril in treatment of senile rheumatic heart disease complicated by chronic heart failure

目的分析卡维地洛联合依那普利治疗老年风湿性心脏病合并慢性心力衰竭的临床效果。方法随机选取我院收治的62例老年风湿性心脏病合并慢性心力衰竭患者参与本次实验,所有病例均选自2015年8月至2017年3月,采取随机抽签的方式将其均分为两组,对照组31例患者采用依那普利治疗,分析组31例采取卡维地洛+依那普利治疗,对比两种治疗方案的疗效。结果相比于对照组患者,分析组患者治疗总有效率更高,心功能改善情况更佳,BNP水平及6MWD更佳,组间差异显著,P<0.05;两组患者不良反应发生率相比P>0.05。结论对老年风湿性心脏病合并慢性心力衰竭患者给予卡维地洛联合依那普利治疗可获得理想的疗效,可有效改善其心功能,且不良反应少,安全性高。

Objective To investigate the clinical effect of carvedilol combined with enalapril in the treatment of senile rheumatic heart disease complicated bychronic heart failure. Methods A total of 62 elderly patients with rheumatic heart disease complicated bychronic heart failure who were treated in our hospital from August 2105 to March 2017 were enrolled and randomly divided into control group and observation group, with 31 patients in each group. The patients in the control group were given enalapril alone, and those in the observation group were given carvedilol combined with enalapril. The clinical effect of these two regimens was compared. Results Compared with the control group, the observation group had a significantly higher overall response rate, a significantly better improvement in cardiac function, and significantly better B-type natriuretic peptide level and 6-minute walk distance （P〈0.05）. There was no significant difference in the incidence rate of adverse events between the two groups （P〉0.05）. Conclusion Carvedilol combined with enalapril has a satisfactory clinical effect in the treatment of elderly patients with rheumatic heart disease complicated by chronic heart failure and can effectively improve their cardiac function, with fewadverse reactions and high safety.
