锥形束CT测量上颌后牙根尖与上颌窦底距离准确性与应用价值研究 被引量:4

摘要 目的探讨锥形束CT(CBCT)测量上颌后牙根尖与上颌窦底距离的准确性与应用价值。方法选取自2016年1月至2018年2月收治的68例接受CBCT检查者为研究对象,总结分析136侧上颌窦、204颗牙齿、952个牙根的CBCT影像资料,观察上颌后牙根尖和上颌窦底的解剖关系,测量上颌后牙根尖和上颌窦底距离。结果上颌后牙各牙根和上颌窦关系分型中A型(59. 6%)最高,第二前磨牙中B型(18. 4%)明显高于E型(8. 1%),差异有统计学意义(P <0. 05),其余各牙根B和E型比例比较虽有一定差异,但无统计学意义(P> 0. 05)。男性和女性恒牙期CBCT头影测量项目中Ptm-A、Go-Pog、GoCo、Pcd-S、U1-SN、L1-NB、FMIA、L1-MP测量值差异较大(P <0. 05),其他指标测量值基本相近(P> 0. 05)。结论 CBCT能清晰观察上颌后牙区的解剖特征,准确分析其和上颌窦底间的关系,距离测量数据精确,在评估上颌后牙区生理病理状况、指导治疗方案等方面的应用价值较大。 Objective To investigate the accuracy and application value of cone beam CT(CBCT)in measuring the distance between the root tip of maxillary posterior teeth and the maxillary sinus floor.Methods A retrospective study was performed on 68 cases of patients who underwent CBCT from January 2016 to February 2018.The CBCT imaging data of 136 sides of the maxillary sinus, 204 teeth, 952 teeth,maxillary root tip were summarized and the anatomical relationship of maxillary sinus,maxillary posterior teeth tip and maxillary sinus floor distance were analyzed.Results The root of maxillary molar and maxillary sinus of Type A (59.6%)was the highest,Type B(18.4%)in second molars was higher than that of Type E(8.1%),and the difference was statistically significant( P 〈0.05),the incidence rate of rest root of Type B and Type E has a certain difference,with no statistically significant difference ( P 〉 0.05).The permanent dentition with CBCT cephalometric measurements in Ptm-A,Go-Pog,Go-Co,Pcd-S,U1-SN,L1-NB,FMIA and L1-MP measured large differences between male and female( P 〈0.05),and other indicators of the measurement value were similar( P 〉0.05).Conclusion CBCT can clearly observe the anatomy of maxillary posterior region,accurate analysis of the relationship between and the maxillary sinus floor,distance measurement data is accurate,in assessing the physiological and pathological conditions,the maxillary posterior region in guiding treatment of great application value,and can be widely used in clinical.
