“8”字双钢缆环扎法在临床髌骨骨折患者治疗中的应用效果

Application Effect of “8” Double Steel Cable Ring Ligation Method in the Treatment of the Patients with Patellar Fracture

目的:探讨"8"字双钢缆环扎法在临床髌骨骨折患者治疗中的应用效果。方法:选取本院2015年1月-2017年10月收治的单侧闭合性髌骨骨折患者59例作为研究对象,按照随机数字表法将其分为对照组(n=29)和观察组(n=30)。对照组采用传统钢丝张力带固定法进行复位治疗,观察组采用"8"字双钢缆环扎固定法进行复位治疗,比较两组手术时间、术中出血量、首次锻炼和骨折愈合时间、术后并发症、临床疗效以及满意度情况。结果:观察组手术时间、术中出血量和骨折愈合时间均明显少于对照组,首次锻炼时间明显早于对照组,差异均有统计学意义(P<0.05)。两组术后感染、创伤性关节炎、膝关节僵硬及粘连术后并发症发生情况比较,差异均无统计学意义(P>0.05)。观察组中优22例、良7例,优等率为73.33%,优良率96.67%;对照组中优13例、良11例,优等率为44.83%,优良率为82.76%,观察组优等率明显高于对照组,差异有统计学意义(X^2=4.965,P=0.026);但两组优良率比较,差异无统计学意义(P>0.05)。随访3个月后,观察组患者满意度为96.67%(29/30),明显高于对照组的72.41%(21/29),差异有统计学意义(X^2=3.815,P=0.026)。结论:"8"字双钢缆环扎法能够减少髌骨骨折患者手术时间及术中出血量的同时,还能加速患者首次锻炼及骨折愈合时间,疗效显著,术后并发症较少,患者满意度较高,值得临床推广应用。

Objective：To investigate the application effect of ＂8＂ double steel cable ring ligation method in the treatment of the patients with patellar fracture.Method：A total of 59 patients with unilateral closed patellar fracture treated in our hospital from January 2015 to October 2017 were selected as the study objects.According to the random number table method,they were divided into control group（n=29） and observation group（n=30）.The control group was used the traditional wire tension band fixation reduction treatment,the observation group was treated by ＂8＂ double cable ring ligation fixation method for reduction treatment.The operation time,intraoperative blood loss,first exercise and fracture healing time,postoperative complications,clinical curative effect and satisfaction were compared between the two groups.Result：The operation time,intraoperative bleeding volume and fracture healing time of observation group were significantly less than those of control group,the first exercise time was significantly earlier than that of control group,the differences were statistically significant（P〈0.05）.There were no significant differences in postoperative infection, traumatic arthritis and stiffness of knee joint and adhesion of postoperative complications between the two groups（P〉0.05）.In the observation group,22 cases were excellent,7 cases were good,the excellent rate was 73.33%,the excellent and good rate was 96.67%;in the control group,13 cases were excellent,11 cases were good,the excellent rate was 44.83%,the excellent and good rate was 82.76%.The excellent rate of the observation group was significantly higher than that of the control group,the difference was statistically significant（X^2=4.965,P=0.026）; but there was no significant difference in the excellent and good rate between the two groups（P〉0.05）.After follow-up 3 months,the satisfaction degree of the observation group was 96.67%（29/30）,which was significantly higher than 72.41%（21/29） of the control group,the difference was statistically significant（X^2=3.815,P=0.026）. Conclusion：＂8＂ double steel cable ring ligation method can reduce the operation time and intraoperative bleeding volume of patellar fracture patients at the same time,can accelerate the first exercise and fracture healing time,the curative effect is remarkable,postoperative complications are less,satisfaction of patient is higer,it is worthy of clinical application.
