空心钉结合支撑钢板应用于PauwelsⅢ型股骨颈骨折的临床疗效 被引量:10

摘要 目的:比较空心加压螺钉与空心钉结合内侧支撑钢板治疗PauwelsⅢ型股骨颈骨折的临床疗效及并发症发生情况,探讨空心钉结合内侧支撑钢板的有效性及安全性。方法:45例pauwelsⅢ型股骨颈骨折的患者,随机分为空心加压螺钉组(20例)和空心钉结合内侧支撑钢板组(25例),比较两组手术时间、术中出血量、术后开始负重时间、完全负重时间、骨折愈合时间及髋关节功能。结果:两组手术时间、术中出血量比较差异无统计学意义(P>0.05),空心钉结合内侧支撑钢板组术后开始负重时间、完全负重时间、骨折愈合时间更短,且Harris评分更高,两组比较差异有统计学意义(P<0.05)。结论:空心钉结合内侧支撑钢板应用于PauwelsⅢ型股骨颈骨折患者与空心加压螺钉同样安全实用,术后固定及加压效果可靠,负重及骨折愈合时间缩短,患髋功能评分更高。 Objective:To compare the clinical efficacy and complications of hollow compression screw and hollow nail combined with medial support plate in the treatment of femoral neck fracture pauwelsⅢ,to investigate the effectiveness and safety of hollow nail combined with inner supporting steel plate.Method:45 patients with pauwelsⅢ type femoral neck fracture were randomly divided into hollow compression screw group(20 cases) and hollow nail combined with inner supporting plate group(25 cases).To compare the operative time,intraoperative blood loss,postoperative loading time,total loading time,fracture healing time and hip function of the two groups.Result:There was no statistically significant difference in operative time and intraoperative blood loss between the two groups(P〈0.05).In the group of hollow nails combined with inner supporting steel plate,the starting load time,complete load time and fracture healing time were shortened after surgery,and the Harris score was higher(P〈0.05).Conclusion:Hollow nail combined with inner support plate is also safe and practical for patients with type PauwelsⅢ femoral neck fracture,the effect of postoperative fixation and compression was reliable,the time of loading and fracture healing was shortened,and the score of hip function was higher after surgery.
