肾上腺嗜铬细胞瘤及异位嗜铬细胞瘤CT和MRI的影像学特征及诊断价值 被引量:10

Imaging Features and Diagnostic Value of CT and MRI in the Treatment of Adrenal Pheochromocytoma and Ectopic Pheochromocytoma

摘要 目的:探讨CT或MRI单独应用及联合应用对肾上腺嗜铬细胞瘤和异位嗜铬细胞瘤的诊断价值。方法:疑似嗜铬细胞瘤患者31例行CT检查,其中11例同时接受MRI检查,以手术后病理检查结果为依据,比较CT、MRI单独应用、或联合应用时对两种嗜络细胞瘤检出率和误诊率,并观察两种肿瘤的CT和MRI特征。结果:CT联合MRI对肾上腺嗜铬细胞瘤及异位嗜铬细胞瘤诊断检出率明显高于CT、MRI单独检查,误诊率明显低于CT、MRI单独检查,差异有统计学意义(P <0. 05); CT检查中,肾上腺嗜铬细胞瘤受压移位发生率明显高于异位嗜铬细胞瘤,肿瘤周围组织部分包绕发生率明显低于异位嗜铬细胞瘤,差异有统计学意义(P <0. 05)。结论:肾上腺嗜铬细胞瘤和异位嗜铬细胞瘤CT和MRI均有相应影像学表现,CT和MRI联合应用能提高2种肿瘤检出率和减少误诊率。 Objective:To investigate the diagnostic value of single use of CT or MRI and combined use of them in patients with adrenal pheochromocytoma and ectopic pheochromocytoma. Method: 31 cases with suspected pheochromocytoma were given CT, of which 11 cases were given both CT and MRI at the same time. The detection rate and misdiagnosis rate of single use of CT or MRI and combined use were compared , and the features of CT and MRI were observed in the two tumors. Results: The detection rate of CT combined with MRI for adrenal pheochromocytoma and ectopic pheochromocytoma was significantly higher than that by single detection of CT or MRI, and the misdiagnosis rate was significantly lower than that of single CT or MRI ( P 〈0.05). In CT examination, the incidence rate of compressive displacement in adrenal pheochromocytoma was significantly higher than that in ectopic pheochromocytoma ( P 〈0.05), and the incidence rate of partial wrapping of tumor surrounding tissue was significantly lower than that in ectopic pheochromocytoma ( P 〈0.05). Conclusion: Both CT and MRI examinations have corresponding imaging findings in adrenal pheochromocytoma and ectopic pheochromocytoma. CT combined with MRI can improve the detection rate and reduce the misdiagnosis rate of the two tumors.
