育龄期女性生殖道高危型HPV感染与微生物组成关联分析 被引量:5

摘要 目的:探索高危型HPV感染的阴道微生物菌群结构与无HPV感染的不同。方法:纳入HPV阳性人数48人,阴性人数50人,采用荧光定量PCR技术扩增测定阴道中常见的15种微生物的相对丰度,采用Metastats方法比较组间差异。结果:根据阴道微生物菌群中的优势菌种可以将菌群结构分为CSTⅠ、Ⅱ、Ⅲ、Ⅳ四种菌型,HPV阳性人群的菌型结构与阴性人群不同(χ2=28. 569,P <0. 001),差异具有统计学意义。其中主要变化为以惰性乳酸杆菌为主的Ⅲ型比例降低而以非乳酸杆菌为主的Ⅳ型比例增多。Metastat组间差异分析表明HPV阳性组惰性乳酸杆菌相对丰度下降,加德纳杆菌、布鲁杆菌、人型支原体、解脲脲原体以及无乳链球菌的相对丰度上升(P <0. 05),差异具有统计学意义。主成分分析结果亦提示,两组人群阴道常见15种微生物组成有显著差别。结论:育龄期女性生殖道高危型HPV感染与其生殖道微生物菌群结构之间具有联系,应进一步开展微生物组学研究,阐明HPV与阴道菌群之间的相互作用,为HPV的预防提供理论依据。 Objective:To explore the difference in vaginal microflora structure between the high-risk human papillomavirus (HPV) positive women and negative women. Methods:There were 48 patients with HPV positive and 50 patients with HPV negative. The real-6me PCR was used to determine the relative abundance of 15 comnlon mi- croorganisms in the vagina, and the Metastats method was used to compare the differences among groups. Results: According to the dominatedbacterium, the vaginal microflora structure was divided into CST Ⅰ , Ⅱ ,Ⅲ, Ⅳ four kinds. The bacterial structure of the HPV positive women was different from that of the negative women, with statis- tically significant difference (x2 =28. 569, P 〈0. 05). The major change was that the proportion of CST Ⅲ whose dominated bacterium was Lactobacillus iners decreased, and the proportion of CST IV whosedominated bacterium was not lactobacillus increased. According to Metastat analysis, the relative abundance of Lactobacillus iners in HPV positive group decreased, and that of Gardneralla vaginalis, Brucella spp, Mycoplasma hominis, Ureaplasma Urealyticum and Streptococcus agalactiae increased, with statistically significant difference (P 〈 0. 05 ). The results of principal component analysis also indicated that there was significant difference in the composition of 15 microor- ganisms between tile two groups. Conclusions: The microflora structure is related with tile high-risk HPV infection in female reproductive tract at reproductive age so further mrobiome research should be conducted to clarify- the interaction between HPV and vaginal flora, in order to provide theoretical basis for the prevention of HPV.
