孕期高血脂性急性胰腺炎大鼠血浆肿瘤坏死因子-α水平变化及意义 被引量:2

Change and significance of tumor necrosis factor-αin pregnant rats with hyperlipidemia-induced acute pancreatitis

摘要 目的探讨孕期高血脂性急性胰腺炎(acute pancreatitis,AP)大鼠血浆肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平变化及与胰腺组织病理评分的关系。方法 48只受孕SD大鼠随机分为对照组和观察组各24只,均高脂饮食喂养。孕第7天,观察组制备AP模型,对照组仅开腹不制备AP模型。分别于孕第1、7、14天检测2组血浆三酰甘油(triacylglycerol,TG)、总胆固醇(total cholesterol,TC)、TNF-α水平,并于孕第14天处死大鼠行胰腺组织评分;Pearson相关法分析血浆TNF-α水平与TG、TC及胰腺组织病理评分的相关性。结果观察组孕第7、14天血浆TG[(1.82±0.44)、(1.83±0.64)mmol/L]、TC[(2.23±0.64)、(2.18±0.54)mmol/L]、TNF-α[(252.49±31.83)、(248.20±44.22)ng/L]高于孕第1天[(0.55±0.21)mmol/L、(0.82±0.52)mmol/L、(78.22±24.10)ng/L](P<0.05);对照组孕第7、14天血浆TG[(1.82±0.56)、(1.79±0.61)mmol/L]、TC[(2.19±0.72)、(2.21±0.71)mmol/L]高于孕第1天[(0.59±0.19)、(0.84±0.51)mmol/L](P<0.05),孕第1、7、14天血浆TNF-α[(78.02±19.44)、(82.11±10.22)、(81.00±18.33)ng/L]比较差异无统计学意义(P>0.05);观察组各时间点血浆TG、TC水平与对照组比较差异均无统计学意义(P>0.05),孕第7、14天血浆TNF-α水平高于对照组(P<0.05);观察组孕第14天胰腺组织病理评分[(9.22±1.29)分]高于对照组[(2.87±0.83)分](P<0.05);Pearson相关分析结果显示,孕第14天,观察组血浆TNF-α水平与TG(r=0.102,P=0.183)、TC(r=0.098,P=0.221)无线性相关,与胰腺组织病理评分(r=0.562,P<0.001)呈正相关。结论孕期高血脂性AP大鼠血浆TNF-α呈高表达,且与胰腺组织病理评分相关,可用于高血脂性AP病情严重程度的评估。 Objective To investigate the change of plasma tumor necrosis factor-α (TNF-α) in pregnant rats with hyperlipidemia induced acute pancreatitis (AP) and its relationship with histopathological score of the pancreatic tissue. Methods Forty-eight pregnant rats receiving high-fat diet were randomly divided into observation group and control group, with 24 rats in each group. On the 7th day after pregnancy, observation group was prepared AP models and control group received laparotomy without preparing AP models. The levels of plasma triacylglycerol (TG), total cholesterol (TC) and TNF-α were detected on the 1st, 7th and 14th day of pregnancy in both two groups, and the rats were sacrificed and the pancreatic histopathological score was recorded on the 14th day of pregnancy. Pearson correlation method was adopted to analyze the correlations of TNF-α with TG, TC and pancreatic histopathologicaI score. Results The levels of TG ((1.82±0.44), (1.83±0. 64) mmol/L), TC ((2.23±0.64), (2. 18±0.54) mmol/L) and TNF-α ((252.49±31.83), (248.20±44.22) ng/L) on the 7th and 14th day of pregnancy were significantly higher than those on the 1st day ((0. 55±0.21) mmol/L, (0.82±0. 52) mmol/L, (78. 22±24. 10) ng/L) in observation group (P〈 0.05). Thelevels of TG ((1.82±0.56), (1.79±0.61) retool/L) andTC ((2.19±0.72), (2.21±0.71) mmol/L) on the 7thand 14thday of pregnancy were significantly higher than those on the 1st day ((0.594±0. 19), (0.84±0.51) mmol/L) in control group (P〈0.05), and there were no significant differences in the TNF-α levels among the time points of the 1st, 7th and 14th day ((78.02±19.44), (82.11±10.22), (81.00±18.33) mg/L) (P〉0.05). There were no significant differences in the levels of TG and TC at all time points between two groups (P〉0.05). The levels of TNF-α on the 7th and 14th day were significantly higher in observation group than those in control group (P〈0.05). The pancreatic histopathological score on the 14th day was significantly higher in observation group (9.22 ± 1.29) than that in control group (2.87±0.83) (P〈0.05). Pearson correlation method showed TNF-α was not linearly correlated with TG (r= 0. 102, P= 0. 183) and TC (r= 0. 098, P= 0. 221), and positively correlated with the pancreatic histopathological score (r= 0. 562, P〈0. 001) on the 14th day in observation group. Conclusion TNF-α is highly expressed in pregnant rats with hyperlipidemia-induced AP, and is closely correlated with the pancreatic histopathological score. It can be used to predict the severity of hyperlipidemia-induced AP.
