子宫肌瘤采取不同手术方式的临床疗效及对术后妊娠的影响

Effect of Different Surgical Treatment of Uterine Fibroids on Postoperative Pregnancy

目的:探讨腹腔镜与经腹子宫肌瘤剔除术治疗子宫肌瘤的临床疗效及对术后妊娠的影响。方法:2015年10月~2016年10月期间,选取某院确诊的子宫肌瘤患者92例,按随机数字表法分为对照组和观察组,每组各46例,对照组患者行经腹子宫肌瘤剔除术,观察组患者行腹腔镜子宫肌瘤剔除术,比较两组手术出血量、手术时间、术后肛门排气时间、住院时间及术后并发症发生率,并随访1年,观察两组术后妊娠及复发情况。结果:观察组手术出血量、术后肛门排气时间、住院时间均低于对照组,手术时间高于对照组,差异有统计学意义(P<0.05);观察组术后并发症发生率低于对照组,差异有统计学意义(P<0.05);两组术后妊娠率、复发率比较差异无统计学意义(P>0.05)。结论:两种手术治疗子宫肌瘤均可取得一定效果,术后妊娠率均较高,但腹腔镜手术在术后恢复方面要优于经腹手术。

Objective： To explore effect of laparoscopic and transabdominal myomectomy in the treatment of uterine fibroids and its cclinical influence on postoperative pregnancy. Methods： 92 cases of patients with hysteromyoma diagnosed in a hospital from October 2015 to October 2016 was selected and divided into the control group and the observation group according to the random number table method. Every group has 46 cases.The control group was treated with transabdominal myomectomy, and the observation group was treated with laparoscopic myomectomy.The amount of bleeding, operation time, postoperative anal exhaust time, hospital stay and postoperative complications were compared between the two groups, and followed up for 1 years. The postoperative pregnancy and recurrence of the two groups were observed. Results： Observation group of intraoperative bleeding, postoperative anal exhaust time, hospitalization time were lower than the control group, the operation time was higher than the control group, the difference was statistically significant （ P 〈0.05）. The incidence of postoperative complications in the observation group was lower than that in the control group, the difference was statistically significant （ P 〈0.05）.The two group postoperative pregnancy rate and recurrence rate had no significant difference （ P 〉0.05）. Conclusion： Two kinds of surgical treatment of uterine fibroids can achieve certain results, postoperative pregnancy rate are both high, but laparoscopic surgery in postoperative recovery is better than abdominal surgery.
