Eotaxin在结核病患者中低表达及其临床意义

摘要 目的:研究探讨嗜酸性粒细胞趋化因子(eotaxin)在不同人群血浆中的表达情况,评价其在结核病诊断中的应用价值。方法:免疫酶联吸附试验(ELISA)检测12例健康人,12例结核潜伏患者,18例结核病患者血浆中eotaxin表达量,并随访其中8例结核病患者抗结核治疗后eotaxin表达的变化。通过绘制ROC曲线,评价eotaxin对结核病的诊断效能。比较9例结核性胸膜炎患者胸水上清和血浆中eotaxin的表达。结果:结核病患者血浆中eotaxin显著低于健康人和结核潜伏患者;Eotaxin在分辨结核病患者和健康对照或潜伏感染者的显示较好的效果,AUC均在0.85以上;结核病患者血浆中eotaxin随着治疗进行逐渐升高。Eotaxin在结核性胸膜炎患者胸水上清中的表达显著低于血浆中的水平。结论:Eotaxin在结核病患者中低表达,能有效地区分结核病,可作为用于诊断结核的生物标识。 Objective: To investigate the expression of eotaxin in different groups of people and evaluate its value in the diagnosis of tuberculosis. Methods: Immune enzyme-linked adsorption test(ELISA) was used to detect the eotaxin expression in plasma of 12 cases of healthy control(HC),12 cases of latent tuberculosis(LTBI) and 18 patients with tuberculosis(TB). Among them,8 cases of TB patients were followed up and their eotaxin expression changes were observed after antituberculosis therapy. Then ROC curve was drawn to evaluate the value of eotaxin in the diagnosis of TB. The expressions of eotaxin in pleural effusion supernatant and plasma of 9 patients with tuberculous pleurisy were compared. Results:The level of plasma eotaxin in TB patients was significantly lower than that of the HC and LTBI patients.The eotaxin showed better results in the differential diagnosis of TB,HC and LTBI patients,AUC all reached over 0. 85 score. The plasm eotaxin level in TB gradually increased after anti-TB treatment. The expression of eotaxin in pleural effusion supernatant of patients with tuberculous pleurisy was significantly lower than that in plasma. Conclusion: Low eotaxin expressions in TB patients are able to differentiate TB,so it can be used as a biomarker in TB diagnosis.
