摘要
目的比较固定与旋转平台膝关节假体治疗重度膝外翻骨性关节炎的临床疗效。方法对76例接受治疗的重度膝外翻骨性关节炎患者按照膝关节假体类型分为观察组(应用旋转平台假体,36例)和对照组(应用固定平台假体,40例)。比较术后两组患者解剖学参数(股骨角、胫骨角、平台后倾角、髌骨高度、膝外翻角)、膝关节屈曲角度和KSS评分情况。结果患者均获得随访,时间6~36个月。两组患者术后股骨角、胫骨角、平台后倾角、髌骨高度以及膝外翻角指标比较差异无统计学意义(P> 0. 05)。术后膝关节屈曲角度观察组高于对照组,差异有统计学意义(P <0. 05)。术后KSS评分两组比较差异无统计学意义(P> 0. 05)。结论旋转平台假体能够有效恢复膝关节屈曲角度,纠正膝关节外翻畸形。
Objective To compare the clinical effect of fixed and rotating platform prosthesis TKA in severe valgus knee deformity osteoarthritis. Methods The 76 cases of severe valgus knee deformity osteoarthritis underwent TKA. All the patients were divided into observation and control group according to instrumental type. The observation group(36 cases) was used rotating platform prosthesis,and the control group(40 cases) was used fixed platform prosthesis. Two groups of postoperative parameters were evaluated,including anatomical indicator (femur angel, tibia angel, posterior slope angle and patellar height,knee valgus angle), knee flexion angle, KSS score. Results All the patients were followed up for 6~36 months. There was no significant difference between two groups of femur angel, tibia angel, posterior slope angle, patellar height and knee valgus angle( P〉 0.05). The knee flexion angle of observation group was higher than that of the control group ( P〈 0.05),and there was no significant difference for postoperative KSS scores between two groups ( P〉 0.05). Conclusions The rotating platform prosthesis TKA can obtain satisfied knee flexion angle, correct the knee valgus.
