无痛管理临床路径在老年股骨骨折患者围术期护理中的应用效果

The clinical effect of painless management in perioperative nursing of senile femoral fracture

目的 探讨无痛管理临床路径举措在老年股骨骨折患者围手术期护理工作中的效果.方法 选取该院2015年5月-2017年5月手术治疗的老年股骨骨折病例92例作为研究对象,以随机分组的方式,将患者分成研究组和参照组,各40例.其中参照组仅实施常规的护理举措,在此基础上,予以研究组患者临床路径指导下的无痛管理模式,分别对两组患者的护理效果情况进行分析比较.结果 通过进行重复测量方差分析结果显示,术后研究组各时间点VAS评分均显著低于参照组,并且随着时间的延长,研究组和参照组的VAS评分呈现出了降低的趋势,差异有统计学意义（均P〈0.05）;对比术后各时间点髋关节功能评分,结果显示研究组明显高于参照组,差异有统计学意义（均P〈0.05）,并且随着时间的延长,两组髋关节功能评分均具有提升的趋势,差异有统计学意义（均P〈0.05）;对比两组患者的总体护理满意度,研究组同参照组相比较,满意度更高,差异有统计学意义（均P〈0.05）.结论 通过应用临床路径为指导的无痛管理模式到老年股骨骨折患者围术期间,能够明显减轻患者痛苦,并且得到较高的护理质量和水平,推动患者康复进程.

Objective To explore the effect of the clinical pathway of painless management in the periopera-tive nursing of elderly patients with femoral fracture. Methods A total of 92 elderly patients with femoral fracture from May 2015 to May 2017 were selected as the research objects and were divided into the research group and the control group according to the random number table method, 40 cases of each group. The control group only carried out routine nursing measures, and on this basis, the painless management mode guided by clinical pathway of the research group patients was given. The results of nursing care were analyzed and compared between the two groups. Results The re-sults of repeated measurement of variance analysis showed that the VAS scores at all time points of the research group were significantly lower than those of the control group, and the VAS score of the research group and the control group showed a decreasing trend with the prolongation of the time. And the differences were statistically significant （ P〈0. 05）. Comparing the scores of hip joint function at each time point after operation, the results showed that the re-search group was significantly higher than the control group, and the differences were statistically significant （ P〈0. 05）. Moreover, with the extension of time, the scores of hip joint function in both groups showed an upward trend and the difference was statistically significant （P〈0. 05）. Compared with the two groups of patients＇ overall nursing sat-isfaction, the research group was more satisfied with the research group than the control group and the difference was statistically significant （ P〈0. 05 ） . Conclusions By applying the painless management mode guided by clinical path-way to the perioperative period of senile patients with femoral fracture, the pain of the patients can be significantly alle-viated, and the nursing quality and level can be higher to promote the rehabilitation process of the patients.
