清肺养阴活血方联合泼尼松龙治疗急性放射性肺炎51例临床观察

Clinical Observation on 51 Patients with Acute Radiation Pneumonitis Treated with Qingfei Yangyin Huoxue Formula(清肺养阴活血方) Combined with Sterane

目的探讨应用清肺养阴活血方联合泼尼松龙治疗放射性肺炎的临床效果。方法 102例放射性肺炎患者采用随机数字表法均分为对照组和治疗组各51例。对照组予以泼尼松龙治疗,治疗组在此基础上加用清肺养阴活血方口服治疗。连续治疗1个月后比较两组临床疗效,治疗前后比较氧化应激指标[包括丙二醛(MDA)、谷胱甘肽过氧化物酶(GSH-Px)、超氧化物歧化酶(SOD)]、免疫功能指标[T淋巴细胞亚群(CD3^+、CD4^+/CD8^+)和自然杀伤(NK)细胞百分比]变化,同时观察不良反应。结果治疗组总有效率(92.2%)较对照组(76.5%)明显升高(P<0.05)。与治疗前比较,两组治疗后血清GSH-Px、SOD浓度均显著增加,MDA水平均显著减低(P<0.01),且治疗组改善更显著(P<0.01)。两组治疗后外周血CD3^+、NK细胞百分比及CD4^+/CD8^+值较治疗前均显著增加(P<0.01),且治疗组上升更显著(P<0.01)。治疗组不良反应率为7.8%,对照组为11.8%,两组比较差异无统计学意义(P>0.05)。结论清肺养阴活血方联合泼尼松龙能提高放射性肺炎患者临床疗效,明显缓解患者症状,改善氧化应激状态,增强机体免疫功能,安全可靠。

Objective To explore the clinical efficacy of Qingfei Yangyin Huoxue Formula（ 清肺养阴活血方）combined with sterane on patients with radiation pneumonitis（ RP）. Methods A total of 102 patients with RP were divided into the control group（ 51 cases） and observation group（ 51 cases） by random number table. Sterane was used in control group,and Qingfei Yangyin Huoxue Formula was used in observation group by oral administration on the basis of control group. The clinical efficacy of both groups was compared after treatment for continuously 1 month. The change of oxidative stress index including MDA,GSH-Px,SOD,immunologic function index including CD3＋,CD4＋/CD8＋and NK cells percentage and adverse reaction were observed before and after the treatment.Results The overall effective rate of the observation group（ 92. 2%） was much higher than that of the control group（ 76. 5%）（ P〈0. 05）. Compared with statistics before the treatment,the level of serum GSH-Px and SOD were obviously higher after the treatment,while MDA were significantly lower in both groups（ P〈0. 01）. The improvement of observation group was more significant（ P〈0. 01）. The proportion of peripheral blood CD3＋,NK cell percentage and CD4＋/CD8＋in both groups were remarkably increased after treatment（ P〈0. 01）,and the rise of the observation group was more significant（ P〈0. 01）. The incidence of adverse reactions of the observation group（ 7. 8%） had no significant difference compared with that of the control group（ 11. 8%）（ P〉0. 05）. Conclusion The combination of Qingfei Yangyin Huoxue Formula and sterane can significantly improve clinical efficacy,relieve the symptoms of patients,improve oxidative stress and enhance immunologic functions in RP patients.
